A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form).

Per your request, a structural and mechanical inspection of the above -captioned
property was made on April 14, 2015. The findings of that inspection, plus recommen-
dations for improvements, are as follows:
The house is a 1930's one-story wood frame structure with asphalt shingle roofs
and wood shingle siding.
Basic construction of the house consists of concrete block perimeter foundation
walls. A built-up wood girder with steel pipe columns support the first floor wood joists
under the center of the house. The joists span wall-to-wall in a series of low -height
crawl spaces under the front, rear and south sides of the house. Locust and other wood
posts support the floor joists under the front porch and rear entry mudroom. Fiberglass
insulation has been installed between the northeast crawl spaces' floor joists. Tar paper
has also been nailed to the bottoms of the joists.
The basement is accessible by an interior stair and an exterior steel Bilco door
hatch -covered stair at the south site of the house. The crawl spaces are not readily ac-
cessible due to their low -height, construction debris and mechanical piping. The areas
could only be partially viewed through small openings in the tops of the full -height base-
ment foundation walls. The structural members visible appear to be generally in fair to

To,prevenf further rainwater entry, which is. corroding th0,furna'ce's,r -btal flue
pipe, a stainless steel rain cap needs to_be°installed on -the top of--the:-ehimney's terra-
cotta flue-.".. A. similar type cap should also -be installed- on the_fireplace's.,flue..to protect
the metal flue .damper. The screened caps will,also prevent an mal; entry and a possible
blockage: _

joists above the furnace and hot-water heater's metal exhaust;fl;ue-pipes.
Note: The fuel oil storage.tank is located in the basement.. There were no-.signs
of leakage. The tank appears to be fairly old, ,with�a large amount of rust. ,Due to, -a.;..-:

pense. It is therefore recommended the tank=be inspected to determine if it needs to be `
replaced. .
Carbon monoxide and fire detectors. need. to be.installed at;the first floor and
basement. The fire detectors are required to be ,installed, in the center halls: and in each
bedroom..Carbon monoxide:: detectors must ;also, be installed in the basement and halls
within 15 -feet of all of the bedrooms. The detectors should be tested periodically to in-
sure that they are.in opefating order and provioing; protection: ,The back-up batteries,
which are needed to keep.them.operating,when there_are.powerfailures,-ahould be

metal grid acoustical tiles. A large number of the. taped seams on the sheetrock are
raised and need to be reworked with joint compound. The plaster, where exposed, has
many cracks and should be covered over or replaced with sheetrock panels. The bath's
metal grid acoustical tile ceiling has water stains and also needs to be replaced.
A number of.the interior doors need to be trimmed to close properly. This evi-
dences that some deflection of the support members is still taking place.
The kitchen and bath fixtures are generally in fair condition and in working order.
This except for the east bath toilet, which appears to need a repair made. Note: The
small -diameter copper tubing gas line for the clothes dryer needs to be replaced with.

metal ducts for fire safety and to conform to present fire Codes.
The living room masonry fireplace is generally in poor condition. The surface of
the bricks in the firebox have spalled. This apparently due to the intensive heat of past
fires. The bricks themselves may not have been fired and cured properly to harden
them when they were manufactured. A mortar parging, which was applied to the firebox
walls, has also spalled off. The metal flue damper at the smoke shelf appears to have
deteriorated due to rainwater entry. This is allowing for a large amount of heat loss from
the house. If a gas log is to be installed, a metal flue liner may need to be installed.
This so that the combustion gases draft properly. It is important that it first be verified
that the fireplace flue does not share the same flue as the furnace and hot-water heater.
An airtight glass door enclosure needs to be installed in front of the firebox opening.

The attic area is accessible by a steep staircase in the northeast bedroom. There
are wood planks on most of the floor joists. The structural members appear to be gen-
erally in good condition. The ridge beam has had a post installed under it for added
support. The roof rafters have long unsupported span lengths. To strengthen them
against wind and heavy snow loads, it is recommended that 2" x 6" collar tie beams be
installed on every third rafter set. They should be located approximately a third of the
