tor to determine if the premises comply with al! applicable codes and ordin-
ances, other than the Building Zone Code, and therefore, no such inspection
has been conducted. This Certificate, therefore, does not, and is not intended
to certify that the premises comply with all other applicable codes and regula-
tions.

Frank Begora and Ethel Begora being duly sworn deposes
and say that we are the owners of property located at Ketcham
Lane and 11000 Main Road, East Marion, NY (Dist. 1000, Sec. 031,
Blk. 04, Lot 29).
The property is approximately one acre in size and
contains four separate buildings, two of which are single family
dwellings. Each of the single family dwellings has a separate
water supply system and sanitary system. The two story one-
family dwelling located at the southerly end of the parcel is in
good condition and presently unoccupied. The northerly most
building is a one-story dwelling and has been occupied year
round as a single family dwelling since the property was pur-
chased in 1956.
The two other structures located in the center of the
property are storage sheds. The smaller structure at one time
was a residence and now used as a store shed. The larger struc-
ture is a legal scallop shop and also used for storage.
A Certificate of Occupancy has never been issued for this
property, and all buildings have existed prior to April 23, 1957
and have been used continuously since that time.

Instructions
A. This application must be filled in typewriter OR ink, and submitte~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
