Instructions
A. This application must be filled in typewriter OR ink, and subn~itted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of ali buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S 9 form or equal).
3. Approval of electrical installation from Board of Fire Under~vriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where app!icabIe.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Dute of any housing code or safety inspection of buildings or premises, or other pertinent inform?

FLORENCE BATTENFELD, being duly sworn deposes and
says:
Your deponent makes this affidavit in support of her
application for a Certificate of Occupancy on pre-existing
dwellings.
Your deponent is the owner of premises located at
(no number) Peconic Bay Boulevard, Laurel, New York, design-
ated on the Suffolk County Tax Map as District 1000, Section
128, Block 006, Lot 07, and more fully described as follows:
BEGINNING at the Northwest corner
thereof at the point of intersection
of the Southerly side of Peconic Bay
Boulevard with the Easterly line of
lands of one Lawrence, formerly land
of Harry G. Young, running thence W.
27 degrees 45 minutes E. along the
Southerly side of said Boulevard 100

Your deponent and John L. Battenfeld owned the above
premises as Tenants by the entirety from 1957 to 1971. In
1971, John L. Battenfeld conveyed his interest in said
premises to your deponent. Thus, your deponent has contin-
uously owned said premises since 1957.
The above described premises contain four buildings
as shown on the annexed survey. Said survey accurately shows
the size and dimensions of the buildings as they presently
exist. The four buildings were constructed prior to April
1957, and consist of a garage and apartment, shed, garage and
house. Your deponent was informed by the previous owner of
