This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage -disposal (S-9 form). Y
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board' Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Dear Sir or Madam:
Please allow this letter to serve as.a formal proposal in an effort to solve what has
become a recurring issue at Patty's Berries and Bunches — a significant lack of usable,
secure storage space. Currently, none of our existing small and portable structures allow
for anything to be locked up and this has proven to be difficult from both economical and
security perspectives.
The berry and flower trades for Patty's Berries and Bunches continue to bloom
and it is with great pleasure that we provide family fun like berry picking and flowers for
the many brides who prefer `Do It Yourself style weddings. However, with each of these
examples, customers request items such as vases and various jars along with other
agriculturally themed token gifts. By not having the necessary secure storage place,
we've been unprofessional along with having the feeling as if our customers were let
down by this service not being offered.
We are kindly requesting your approval for Patty's Berries and Bunches to place a
prefabricated 12ft x 24ft shed on the property outlined within our respective Building
Application Permit. The shed, being far less than the maximum size afforded by §72-8 of
Chapter 72, would allow us to securely store such items as our point of sale systems
(iPads, credit card machines and other communication devices) along with other
products. It would also help sell items like vases, planters, bird feeders, jams, honey and
other vintage inspired, agriculturally themed gifts fully related to the produce we grow
and in compliance with §72-7 of Chapter 72. While we aspire to one day hopefully build
an entirely new farm stand, this plan will fit within our current budget and still help our
troublesome issues.
We are hardworking locals residents and would sincerely appreciate your
assistance in approving this proposal — it would allow our establishment the opportunity
to stay above water and hopefully grow to help keep up with many of the newer and
larger farm stands.
