Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to Al~ril 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusua~ natural or topographic features, 2. That the structures, as shown on the acconpanying copy of the July 12, 1972 survey, are "pre-existing", and That deponent knows of his own knowledge that tile premises are used solely as residence premises, and have bccn since the construction. This affidavit is made on behalf of the owner, who is in the process of obtaining a mortgage on the premises (your deponent represents her in this transaction) and who, by your deponent, is applying herewith 26th day of January, 1988. for a certificate of occupancy on pre-exi~g dwelling.
