A. This application must be filled in typewriter OR ink, and submitted m~ to the Building Impel-
tot with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic fceture~
2.Final approval of Health Dept. of water supply and sewerage disposel-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, · certificate of Code compliance from the Architect or Engineer responsible for the building.
· 5. Submit Planning Board approval of completed site plan requirements where applicable.

LAURENCE A. LaVEGLIA, being duly sworn, deposes
and says:
That deponent Laurence A. LaVeglia, resides at
475 Wood Lane, Greenport, New York 11944. The undersigned
completed the construction of his dwelling at 475 Wood Lane~
Greenport, New York 11944 in the year 1956. At that time
it was not necessary to obtain a Certificate of Occupancy
for a dwelling from the Building Department of the Town of
Southold.
Deponent knows of his own knowledge that the one-
family house known as 68 Wood Lane, Greenport, New York
which is located less than two hundred (200) feet south of
deponent's dwelling, had been completed and occupied at
the time the deponent was constructing his own dwelling and
had been
so occupied
