As a courtesy we have made the initial review of the above referenced site plan application to establish the completeness of the submission. Conceptually,the proposed appears to be more in keeping with zoning and the site plan application requirements however we cannot process this application further until we receive a complete application. In order to move forward with the review of this site plan application you will need to first formally submit a complete application that contains of the requested items in the October 1 ,2008 letter including a revised notice of disapproval (since you have changed the design of the building from one to two buildings), completed Long EAF, application fee ect. I have attached a copy of the October 1 letter as a reference. Conceptually the revised plan looks as if you may have included the remainder of comments from the letter but, it is difficult to read as only partial of the plan was attached to the email. So you will also need to submit 7 copies of the revised plan with the remainder of the information requested. Once you submit the information, your application will be reviewed for completeness then scheduled for a work session whereas the Board will decide to formally accept your application for review. Once this step happens your application will be referred to interested agencies and Town Departments for review as required by Town Code. Prior to an approval of the site's access and issuance of a New York State Highway Work Permit, the following items must be addressed: 1. We cannot provide a complete review of your submission without a plan sheet showing all existing and proposed pavement markings and signs. We recommend that you consult our site plan requirements check list which is contained in our driveway policy manual which is available on our website at www.nysdot.gov. On the NYSDOT A -Z Index click on "H", then click on Highway Work Permits and then click on Policy and Standards for the Design of Entrances to State Highways. Additional comments may be forthcoming once you provide us with plans that show all required features. 2. In recent years, many states have employed access management as a major technique to address conflicts between through traffic and that generated by developments. The goals of access management are to limit the number of access points, separate conflict points and remove turning traffic from through movements. New York State is utilizing this technique to minimize impacts to State highways. We recommend, therefore, that cross access to this site be obtained from adjacent property. If this is not presently possible, it should be shown on the plans for implementation as part of future redevelopment. • Ms. Denise Heyse December 16, 2008 Page 2 of 3 3. Curb, sidewalk and pavement repairs should be added with all details referenced to NYSDOT specification items numbers. 4. Please show how existing drainage will be adequately addressed. Also, a driveway profile or on site elevations, including elevations at the State highway right-of-way line, shall be shown on the plans to ensure that all drainage is contained on site since we do not permit runoff from private property onto the State highways. A note shall be added to the plans requiring the contractor to clean existing drainage basins along and immediately adjacent to the NY 25 site frontage at the completion of construction. 6. We will require the entire width of the NY 25 shoulder be re -constructed for a minimum of the driveway width plus twenty-five (25) feet on each side of the driveway. The limits for shoulder re -construction shall be shown on the plans and it shall include full depth asphalt construction. Tack coat shall be included between every asphalt lift and at the joints between new and existing asphalt pavement edges. Please include the appropriate detail and NYSDOT specification item number. The asphalt item numbers and depths shall conform to current superpave specification item numbers as follows: Top Course - 1-1/2", Item 402.098301M & Item 402.098311M Binder Course - 2-1/2", Item 402.198901M & Item 402.198911M Base Course - 6", Item 402.378901M & Item 402.378911M Sub Base Course — 6", Item 17304.101199M 7. A Work Zone Traffic Control plan must be provided for work on NY 25. The plan must include the note "All lanes must be open to traffic before 10 AM and after 3 PM. No lane closings are permitted on weekends or holidays. Nighttime lane closings will not be permitted without prior approval from the State Permit Inspector." 8. The plan must include the note "Repair existing shoulder, sidewalk and curbing as ordered by State Engineer." 9. Please show and call out the nearest NY 25A mile marker on the plan. 10. We recommend that permanent survey markers be installed at the property limits along the State highway frontage. Please call out these locations on the plans and include the appropriate detail. 11. Any utility work proposed in State Highway right-of-way will require separate application and submission of plans (installation details, restoration details and Maintenance and Protection of Traffic plan - all referenced to NYSDOT specification item numbers and the Manual of Uniform Traffic Control Devices) to our Riverhead Maintenance facility. The applicant may contact Mr. Kevin Matthaei at (631) 727-1731 for further directions regarding Utility Highway Work Permit (HWP) applications. The applicant should be made aware that utility HWP issuance is subject to issuance of the HWP required for site work. Please submit four (4) sets of revised plans.
