This application must be filled in typewriter OR ink, and submitted in triplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approvai of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit P~anning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey o~ property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Anrig, the decedent was the owner of real property located on the south side of Main Road, diagonally across the street from Mattituck High School. Said premises was sold on April 27, 1979 by your deponent as the Administrator of the Estate to Stanley J. Waimey, who intends to use the premises for the purposes of a real estate office. That your deponent knows of his own knowledge that the premises has been continuously used for business and office purposes since prior to 1957. That the last tenant in the premises was a Mr. Gorman who ran a tax business from said premises, Mr. Gorman having vacated the premises on May 1, 1978. Prior thereto a Mr. Acroyd occupied the premises operating an advertising business from said premises. That prior thereto a Mr. A1 Schwicker operated the premises as a gas station, luncheon and candy store and sundries business and his occupancy of said premises was since approximately 1945. That the property in question has been continuously used for business and office purposes prior to 1957 the date the Southold Town zoning ordinance was enacted. Anrig unoccupied Upon receipt of an application for a Pre-Existing Certificate of Occupancy I made an inspection of the buildings at the above captioned premises. Accompanied by Stanley J. Waimey I began this inspection at approximately 11:45 A.M. The dwelling is a one story wood framed structure constructed on a partial cement block foundation for middle part of building with west side constructed on cement h lock piers and east rooms constructed on wood posts. The building contains a living room, kitchen, toilet room, half bathroom (toilet & lavatory) and two rooms on the east end of building. Heat is furnished to building from two gas fired space heaters one located in living room at west end and one in east room of building. Accessory structure on east side of premises is a wood framed one car garage with work shop. The following violations of the Housing Code, Chapter 52, Code of the Town of Southold, N.Y. were found: Toilet room, half bath, no moisture resistant floor covering. krticle II, Section 52-26 A 4. PlumbinE Facilities - no tub or shower facilities within dwelling. Article V, Section 52-51D 2 c. Sewage - Fresh air vent for buildings sewage system terminates within crawl space of buildings foundation. Article V, Section 52-51C 8. Electrical - wood panel containing electric service panel & meter is loose and is in a deteriorating state from destructive insects. Article V, Section 52-56 A 1 & 2. Open junction box in cellar for electrical water pump connection. Article V, Section 52-56 A 1. - Electrical splice of conductor without proper junction box at same location of opem junction box. Article V, Section 52-56 A 1. Shut off valves - gas cooking range - no shut off valve at unit. Article V, Section 52-52 B 2. Exterior protection accessory building - roof has excessive openings, glass missing from windows and garage door - frame deteriorating.
