being duly sworn, deposes and says:
I am over the age of eighteen years and reside at 812
Park Avenue, New York, New York.
I am a co-owner of property bearing Suffolk County Tax
Map numbers 1000-1-1-3.3 and 3.1 (partial), which
property is depicted on the attached survey of Chan-
dler, Palmer & King, dated December 15, 1989. I am
very familiar with the structures located on that prop-
erty. The property was acquired by my parents in 1952
and has been owned continuously since that date by me
or members of my family, either directly or indirectly
through a corporation owned and controlled by me or
members of my family.
All of the structures shown on the survey were fully
completed before 1957 and have remained in their cur-
rent location without expansion since that date, except
for (i) a bathroom addition to the main dwelling and a
small accessory structure near such bathroom addition,
each of which was completed in or about 1984 under
Building Permit No. 12784, (ii) an inground pool, which
was completed during 1989 under Building Permit No.
18167 and (iii) a poolhouse, which was completed during
1989 under Building Permit No. 18378. The structure to
the south of the main dwelling identified as a "W/F
Cottage" on the survey is actually a two-car garage
with 2 one-bedroom apartments.
I make this affidavit knowing full well that the Town
of Southold Building and Zoning Department will rely
upon the facts as stated herein to issue a pre-existing
Certificate of Occupancy for said structures to Mark E.
Andrews and Elizabeth Quay Andrews for a single-family
dwelling, detached two-car garage with one-bedroom
apartment, and detached garage A /
