AFFIDAVIT STATE OF NEW YORK ) ss.: COUNTY OF SUFFOLK ) ANN~ FLAHERTY, being duly sworn, deposes and says: I reside at 415 Sigsbee Road, Mattituck, New York, and I submit this affidavit in support of the Application of AMJAD Associ- ates for a pre-existing use certificate of occupancy for the premises known as no# Sigsbee Road, Mattituck, New York, SCTM #1000-143-1-6, now owned by AMJAD Associates, and formerly owned by Anne E. Buono. I have resided on Sigsbee Road for over thirty years and I am fully familiar with the subject premises. I have compared the subject premises with a survey of the pre~.~ses entitled 'Burvey of lots 8 & 9, amended map of Mattituck Park Properties, Inc.,"dated July 22, 1985 prepared by Peconic Surveyors and Engineers, P.C. Except for an addition which was added to the southerly side of the subject premises during 1974, the above-referenced survey depicts the premises as it existed prior to April of 1957. Other than the addition made to the southerly side of the pre- mises in 1974, no other additions or modifications have been made to the premises since 1957. This affidavit is made with the knowledge that the Building Department of the Town of Southold will rely upon the truth of the statements made herein.
