Original and two (2) copies of Town of Southold Planning Board Site Plan Application; 2. Original and two (2) copies of Applicant's Affidavit; 3. Original and two (2) copies of Applicant's Transactional Disclosure Form; 4. Elite Towers, LP Check No. 1328 in the sum of$2,000.00, which sum represents the fee for new facility, pursuant to Town Code § 280-74A(2)(b); 5. Three (3) counterparts of Environmental Assessment Form, prepared by VHB Engineering, Surveying and Landscape Architecture, P.C, dated January 16, 2015; 6. Three (3) counterparts of LMW Consistency Assessment Form, prepared by VHB Engineering, Surveying and Landscape Architecture, P.C.; 7. Seven (7) Copies of Notice of Disapproval from the Town of Southold Building Department, dated October 22, 2014; 8. Seven (7) copies of completed Building Permit Application, filed with the Town on September, 26, 2014, including the following supporting documents: a. Seven (7) copies of Engineering Report, prepared by Scinetx, LLC, dated September 4, 2014, demonstrating the project's compliance with the Maximum Permissible Exposure Regulations, pursuant to Town Code §§ 280-74C(l), 280- Town of Southold Planning Board Mattituck Fire District Site Plan/Special Exception Application January 21, 2014 Page 2 of 3 74B(1)(a) and 280-70K; b. Seven (7) copies of Structural Certification Letter, prepared by William R. Heiden, III, P.E., of Valmont Industries, Inc., dated September 19, 2014, documenting the proposed facility's structural compliance with local, state and federal codes pursuant to Town Code §§ 280-74C(1), and 280-74B(1)(b); c. Seven (7) counterparts of Applicant's FCC Antenna Structure Registration and FCC Licenses pursuant to Town Code §§ 280-74C(1) and 280-74B(1)(c); d. Seven (7) copies of FAA Determination of No Hazard to Air Navigation, dated June 13, 2013, along with copy of Extension Letter, dated November 14, 2014, pursuant to Town Code §§ 280-74C(1), 280-74B(1)(c), and 280-74C(2); e. Seven (7) copies of Statement of Andrew J. Hintze, Integrated Wireless Technologies, LLC, dated April 28, 2014, together with accompanying radiofrequency gap and propagation maps, pursuant to Code §§ 280-74C(1) 280- 74B(1)(d) and (e); f, Seven(7) copies of Deeds,pursuant to Code §§ 280-74C(1) and 280-74B(1)(g); g. Seven (7) copies of the copy of the fully executed lease agreement between Mattituck Fire District and Elite Towers, L.P., dated May 16, 2011, pursuant to Town Code §§ 280-74C(1) and 280-74B(l)(g); 9. Seven (7) counterparts of Visual Analysis, prepared by VHB Engineering, Surveying and Landscape Architecture, P.C, dated January 2015, pursuant to Town Code § 280- 74C(3); 10. Nine (9) sets of Zoning Drawings, including Site Plan, Vicinity Plan and Elevation Drawings, prepared by prepared by Gary Musciano, RA Architect, Revision 10, dated September 3, 2014, addressing Town Code §§ 280-74C(4), 280-74C(5), 280-74C(9), 280-74C(I0), 280-74C(I 1), 280-74C(12), 280-74C(13) and 280-133A(4); 11. Two (2) sets of Photographs of existing pole, pursuant to Town Code § 280-74C(8); 12. One copy of the Building Permit Application, pursuant to Town Code § 280-741)(1)(a); 3. Once copy of the Site Plan Application, pursuant to Town Code § 280-74D(1)(b); 4.
