This application must be filled in typewriter OR ink, and submitted in DGIPLICATE to the Building Inspector with the following; for new buildings or new use: I. Final survey of property with accurate location of oil buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Cede compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Ackroyd & wf. occupied: owner Upon receipt of au application for a Pre-Existing Certificate of Occupancy I made an inspection of the buildings on this premises. Accompanied by Curtis Horton we were admitted to dwelling by Robin Ackroyd who accompanied us on this inspection that began at approximately 3:20 P.M. The dwelling is a wood framed two and one half story structure constructed on poured concrete foundation. First floor contains a kitchen with eating area, dining room, living room with a fireplace, den with a fireplace, front entry vestibule and entry hall with prime stairway providing access to second floor. A secondary stairway to second floor is accessible within pantry. Second floor has five bed- rooms, a TV room and two full bathrooms off hallway. Third floor has two bedrooms and a full bathroom. Accessory building in rear yard is constructed of cement block and is being used for storage and stabling one horse. The following violations of the Housing Code, Chapter 52, Code of the Town of Southold were found: Dwelling: Ent~: to kitche~ ~o ~e~s of controlling light in room at point of entry. Armic e , Section 52-56 B.2. Stairway: secondary, no means of conrolling light for safe descent. Article II, Section 52-26 B.3. Stairway: third31anding, ceiling light fixture not operating for safe ascent and descent of stairway. Article II, Section 52-26 B.3. Bedroom: third floor, west end of building, loose and missing ~ck ceiling of dormer. Article III, Section 52-32 C. Bathroom: third floor, door broken, does not provide privacy. Article II, Section 52-26 A.1. Stairway: first floor to cellar, open side requires hand ~ail. ~rticle II, Section 52-27. Furnace: draft control on smoke Stack stuck in an open position. Article I, Section 52-15 New York State Building Construction Code A505-2a. -2- Plumbing: Dishwasher drain connected to cast iron soil pipe no water seal for fixture. Article V, Section 52-51 C5. Two water pipes leaking near domestic hotwater heater in cellar, one at tee of half inch pipe, one at elbow of three quarter inch pipe. Article V, Section 52-51. Structural: front entry open roofed over porch-south corner settling and does not provide adequate support of post supporting roof, one of center posts rotted away at base. Article III, Section 52-30 A.B. In view of antiquated electrical wiring within dwelling an electrical inspection is being requested by N.Y. Board of Fire Underwriters.
