APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
:3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industria~ buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April lg57), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
:3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

That the undersigned do(es) hereby give consent to the Building Inspectors
of the Town of Southold to enter upon the above described property, including
any and all buildings located thereon, to conduct such inspections as they may
deem necessary with respect to the aforesaid application, including inspections
to determine that said premises comply with all of the laws, ordinances, rules
and regulations of the Town of Southold. The Real Estate office is open during
business hours.

Deponent is a member of the firm, Wickham, Wickh~n & Bressler, P.C.
with an office at Main Road, Mattituck, New York; and is the attorney for
Agatha Abatelli, who makes this application for a certificate of pre-existing
use on part of the Abatelli Real Estate office, Main NDad, Cutchogue, New York.
The property was purchased by Philip W. Abatelli in 1956 fr~n Olin and LeBoy
Glov~r and in~rediately thereafter, before April, 1957 the Real Estate Office
was constructed. The addition, however, was erected in 1982, for which
building permit % 11763Z was obtained on June 30, 1982. Application is made
