The resellers of Motor Fuel (resellers) listed below are authorized to issue Pre-
liminary Certifications of Prepayment of Sales Tax and Payment of Motor Fuel Tax
as of the date above. This list is subject to monthly revision as additional
resellers receive permission to issue Preliminary Certifications and other re-
sellers no longer have the authority to issue them. Updated lists will be mailed
monthly to all FT-943 and FT-945 filers. -
Any purchaser who makes purchases from a reseller and who had been given a copy
of an Authorization to Issue Preliminary Certifications, should verify that the
supplier's name is on this list of resellers authorized to issue preliminary
certifications. If you do not find the name of that supplier on the list or
fail to verify that-the name is on the list, you will be considered not to have
accepted in good faith the Authorization to Issue Preliminary Certifications.
If your supplier presents an Authorization to Issue Preliminary Certification,
and is not on the list, you should write to the Fuel Tax Unit, Building #9,
Room 402A, W. A. Harriman Campus, Albany, New York, 12227, to determine if the
seller has been authorized to issue Preliminary Certifications, since the publication
date of the list. Verbal responses may be obtained by calling (518) 457-4397.
However, this will not protect you in having accepted the document in good faith.

IF YOUR SUPPLIER, AT THE TIME_ OF DELIVERY OF THE MOTOR FUEL TO YOU, MEETS EITHER
ONE OF THE FOLLOWING CONDITIONS, THEIR NAME NEED NOT APPEAR ON THE LIST ON THE
REVERSE SIDE OF THIS FORM.
1. A properly completed Certificate of Prepayment of
Sales Tax and Payment of Motor Fuel Tax (Form FT-935)
is given to the purchaser at the time of delivery of
motor fuel
OR
2. An invoice or other billing document given at the
time of delivery which contains the following, will
be acceptable as certification of prepaid tax:
-Seller's name and address
-Seller's sales tax identification number and
motor fuel registration number, if any
-Pu;chaser's name and address
-Type of fuel purchased (leaded, unleaded, premium)
-A separately stated amount per gallon for each tax
included in the selling price.
-A signed statement that the amounts of tax shown
have been or will be paid by the seller or his
-'' supplier. If the seller is a registered motor
fuel distributor under Article 12-A, no signature
- will be required. If the payer of the motor fuel
tax is other than the seller, that payer must be
named.
If your supplier does not meet either of the above conditions, then his name
must appear on this list. In addition, the supplier must also furnish you with
a copy of his Authorization to Issue Preliminary Certification of Prepayment
of Sales Tax and Payment of Motor Fuel Tax (Form FT-952) valid as of the date of
your delivery.
Please consult publication-N-85-25 and N-85-48 for other requirements relative to
the purchasing of Motor Fuel by use of a Preliminary Certificate.

PRELIMINARY CERTIFICATES OF PREPAYMENT OF SALES TAX _� t
AND PAYT4EEN;T OF MOTOR-FUEL TAX -
During November 1985 the Tax Department will issue authorizations to issue Preliminary
Certifications of Prepayment of Sales Tax and Payment of Motor Fuel Tax (Authorizations) .
These will be issued for a period of up to one year and will expire one month after the close
of the applicant's fiscal year.
These authorizations will be issued upon application to the Tax Department, provided the
applicant complies with all of the requirements of Notice N85-25 and the issuance of such
authorization will not imperil the collection of the Sales or Motor Fuel Taxes. If the
applicant is a registered Motor Fuel Distributor and the application is approved, notice of
approval will be sent to the applicant detailing the conditions of the authorization. If
the applicant consents to such conditions, he should return a signed copy of notice of approval
to the Tax Department. The authorization will then be issued. If the applicant is not a
registered Motor Fuel distributor, he must provide the Tax Department with names of the
distributors who will be paying the tai: to the Commission. Approval will be granted only for
purchases from those suppliers that the applicant has advised will be paying the tax.
Renewals
Thirty days prior to the expiration of the authorization, the applicant must reapply
to the Tax Department demonstrating that he needs to continue to issue preliminary

CONDITIONS OF USE
Seller -�
Based on the information you furnished to the Department of Taxation and Finance, you
are authorized to issue preliminary certifications only for deliveries made under those cir-
cumstances and from those locations identified in your request for authorization to issue
preliminary certifications.
Purchaser
As a purchaser you may only accept preliminary certifications from the named seller for
purchases made during the period in which this authorization is valid.

the detail of the background seal is completely visible may be reliE.d upon by customers when
accepting preliminary certifications.
Prior to the first sale during the period covered by this document, a seller must
furnish a copy of this document to his customer. The purchaser must retain a copy of this
authorization for a period of thirty (30) days beyond the expiration date of the authorization.

ONDITIONS OF ISSUANCE
Preliminary certificates may only be issued under the following terms and conditions:
tegistered Motor Fuel Distributor
IN ADDITION TO any other requirements set forth in Notice N85-25, the authority to issue
preliminary certifications is subject to the following conditions:
i. Preliminary certifications may be issued only when, due to the circumstances of the
delivery, it is not possible to provide a complete certification at the time of delivery.
Additionally, preliminary certifications may be issued only in those circumstances
specified in your request_ for authorization to issue preliminary certifications and
approved herein.
'. A complete certification in accordance with Notice N85-25 must be furnished to your
customers within 10 business days of the delivery of motor fuel.
3. If, for any reason, your registration as a distributor under Article 12-A of the Tax
Law is discontinued, vour authority to issue preliminary certification is simultaneously
and automatically revoked.
4. A copy of your Authorization to Issue Preliminary Certifications of Prepayment of
Sales Tax and Payment of Motor Fuel Tax must be furnished to each of your customers to
whom preliminary certifications will be issued.
5. Failure to issue a copy of your authorization by the time the preliminary certification
is given will preclude the customer from claiming he accepted the preliminary certification
and the underlying motor fuel in good faith.
i. Failure to comply with any of the conditions stated herein and in Notice N85-25 results
in immediate revocation of your authority to issue preliminary certifications.

In order to protect pur.cha,ers of Motor Fuel, the department will publish a list of
Motor Fuel Resellers aut:,arized to issue Preliminary Certifications. This list will be
)ubiished- as of the first (1st) of each month and mailed to all petro:'.eum sellers with forms
17-943, or FT-945 (all motor fuel sellers are required to file either one or both of these
norms) .

SPECIFICATIONS FOR NO. 2 FUEL OIL
FOR THE TOWN OF SOUTHOLD
Under these specificastions the Town of Southold seeks bids for furnish-
ing all of the heating fuel oil (No. 2 fuel oil) required for the heating of the
Town buildings consisting of the Southold Town Hall, Main Road, Southold,
New York (1,500 gallon tank) ; the Town Highway Department building, Peconic
Lane, Peconic, New York (550 gallon tank and 275 gallon tank) . Bidders are
to submit bids for furnishing all of the necessary fuel for the calendar year
1986. Deliveries are to be made to the Town buildings as and when required.
Bid price shall be based upon Northville Industries Corporation's Long
Island tank car consumer rack price, to which has been added the delivery
cost per gallon. The lowest rack price within five (5) days of each delivery
will be used. The aforementioned tank car consumer rack price is published
daily in the Journal of Commerce, or may be obtained by calling Northville
Industries Corporation, Melville, New York (516)293-4700.
