Members Present: Councilman Bill Ruland, Town Engineer Jamie Richter, Planning Dep't Director Heather Lanza, Chief Martin Flatley, Margaret Brown, Frank Field, Greenport Rep Bob Feger, Highway Superintendent Vincent Orlando, Jim Baker, Rona Smith and Tom Fox. Minutes of May 13, 2014 and June 23, 2014 A motion to approve the minutes was made, seconded and was carried. Certificates of Recognition —April 29, 2014 Frank Field received his certificate issued by the Town Board. EETC Update That organization met on July 23, 2104 at Riverhead Town Hall. With respect to the issue of bus shelter maintenance and snow removal in Southold and Greenport, Heather Lanza reported that those matters would be considered by the EETC. Public Requests Signage at Laurel Lane & Peconic Bay Boulevard Neb met with the local resident who had requested signage cautioning joggers, walkers and bicyclists to observe traffic rules at the above intersection so as to avoid accidents with motor vehicles. Vincent Orlando observed that signage cautioning them about the "wrong side" of the roadway may be appropriate to encourage self-regulation. Chief Flatley will review a roadway signage book for guidance. Margaret Brown will contact the resident with an update on the situation. Neb will check with the Town Engineer and Highways Superintendent concerning appropriate signage. Route 25 Crossinq at Handy Pantry, Mattituck The NYSDOT's East End Acting Regional Traffic Engineer wrote to the local residents who had requested traffic safety measures to advise that an investigation at that location will be conducted and that they would subsequently be advised of the findings. Crosswalk Request by the Cutchogue F.D. on New Suffolk Road So as to improve the safety of its firefighters responding to alarms and also for the public attending various CFD-related functions, the CFD has requested the placement of a crosswalk in way of its parking lot at the above location. Neb will send a memo to the Town Board (with a copy to the CFD) concerning that issue and the necessary funding for implementation. Bicycle Travel Concern, Soundview Avenue & Horton Lane, Southold In its letter to the Highway Superintendent of July 18, 2014, the Kenny's/McCabe's Beach Civic Association requested the consideration of signage to be installed on Soundview Avenue on the east and west sides of Horton Lane at an appropriate distance from that intersection so as to warn cyclists of the stop signs there. The Association is concerned about the apparent failure of cyclists to observe those stop signs. Margaret Brown will contact the Association with a situation report and Chief Flatley will consult his signage book for guidance. Southold Town Fire Chiefs Council Concern —SR25, Greenport The above organization's letter of June 26, 2014 to the NYSDOT expressed its concern about the state of the roadway on SR25 (Main Street) between Manor Place and CR48. The Council also seeks the immediate repair of the hazardous roadway in that area. Jamie Richter will contact NYSDOT for clarification concerning which municipality or agency is responsible for that roadway maintenance. Supervisor& Town Board Requests Traffic Light at Route 25 & Peconic Lane, Peconic& Planning Board Recommendations Heather Lanza will follow up with the Planning Board, as its forthcoming request for the Commission's review of that traffic control issue is still outstanding. Heather also reported that Hampton Jitney, in agreement with the Greenport Brewery, will move its westbound stop at Peconic Lane 20 feet further to the East. No Throuqh Trucks— King Street, New Suffolk Due to the lack of a quorum at the July 2014 meeting, the matter was tabled for discussion today. Chief Flatley advised that he would draft an SPD letter to local merchants concerning the above issue. Reconsideration of Stop Sign/Pedestrian Crosswalk, Peconic Bay Boulevard, Mattituck The Commission reviewed the June 24, 2014 letter from the Marlene Lane Civic Association seeking reconsideration of that issue. Chief Flatley reviewed the situation at that location and recommended that, as there are good sightlines at Marine Lane and PBB, no further action is required. The Commission unanimously agreed with that suggestion. Time Limit Concern, Southold Public Parking Lot/ East of Wells Avenue The Supervisor forwarded a letter of July 9, 2014 from a local resident concerning the ticketing of vehicles parked at that lot for more than 2 hours. The Supervisor also forwarded a copy of Town Code Section 260-12, which confirms that a 2-hour limit does apply at that lot. It was suggested that longer term parking is available at the Traveler Street lot. Planning Board Requests Town Comprehensive Plan Heather Lanza reported that issuance of the Transportation & Infrastructure Chapter has been delayed to December 2014. Signage Concern, Intersection of Boisseau Avenue and CR48, Southold Due to the apparent confusion generated by the Yield and Stop Signs at that traffic circle, Heather Lanza will draw a map of that location for the Commission's review. Current Transportation Issues - Updates Project Bus Stop SCDPW has no problem with the recommended placement of the shelters and has funds available for the project. However, the project is subject to NYSDOT's approval for placement on SR25. Orientation/Interpretive Signage Project— RBA Phase II RBA will supervise and inspect the placement of the signage. Handicapped Parking Request—Southold Pharmacy, Route 25, Southold The appropriate signage has been placed at that location. The Town of Southold attorneys have completed an intensive search and have exhausted all avenues to find a dedication letter or resolution stating this portion of NYS Rt 25 to the town of Southold. In the past as all good neighbors do in small towns,we help each other out by fixing pot holes or Xppl�ikri7some sand during a long winter,but this doesn't constitute ownership. As per the attached letter from the Southold Town Fire Council,all the fire departments use Suffolk County road 48 east bound and NYS route 25 west bound to access main road (NYS Rt 25) south bound to transport people to the only Hospital in our town ELIH. I would hope while under taking your vast repaving project(PIN0042.37)which resumes at RM 25 0704 1670 Knapp street Greenport as per your attached drawing (LP-01)you could be so kind as to add this small designated section of State road Rt 25 south to Manor place Greenport. With this serious responsibility in mind, I am writing to alert you to the hazardous road surfaces on upper Main Street(State Route 25)in both the Village of Greenport and the Town of Southold lying between Manor Place and County Road 48, and to request an immediate repair thereof. Although this stretch of road spans only 1800 feet, it is the route used by almost every one of our When transporting patients to Eastern Long Island Hospital. Over the years this road surface Deteriorated to a point where travel had become difficult for both our patients and our ambulances, however the winter of 2013/2014 extracted a terrible toll and what had been previously difficult is now patently dangerous, particularly when transporting criticalally ill patients or those with spinal injuries. The Chiefs'Council is asking you to do everything within your power to effectuate an emergency Resurfacing for this road which is a vital link between our communities and our only hospital. Firemen responding to alarms must cross the street to access the emergency vehicles in the fire house. The speed of the traffic in this area has gradually increased over the years and this year has reached a point, in my estimation, of being dangerous. In addition, alarms, various fire department functions, fund raising activities, and public functions require both fire department members and members of the public to cross from the parking area to the fire house at various times during the day and at night. With large functions our Fire Police normally police the crossing area and control the traffic both vehicular and pedestrian. A crosswalk would be an advantage as well as an additional level of control. However in the normal course of events firemen and pedestrians must cross the street without any aid. We agreed that no further action is necessary. We also decided that no discussion of Town Code 260-12 is warranted by the Town Board since the imposition of the time limit was to protect access to essentially the downtown merchants' parking lot. In addition, Ms. Amara is not correct in her statement the "there is no place to park in the village for more than 2 hours without getting a ticket." Russell, Supervisor Date: July 16, 2014 Re: Southold Municipal Parking Lot The attached letter was received on July 14, 2014 from Ms. Cheryle Amara regarding the public parking lot in Southold, approximately 170 feet east of Wells Avenue. The issue is addressed in the Town Code, 260-12. A copy of the code is attached for your reference. Please review the time limit concern for the municipal parking lot locations and see if it warrants further discussion by the Town Board. Thank you for your attention to this matter. July 9,2014, Dear Mr. Russell, Chief Flatley and Mr. Orlando, I wasn't quite sure who I should send this letter to, so I decided to send it to the three of you in the hopes that one of you will be able to look into the issue. This past February I had to go into the city for a doctor's appointment. My daughter was meeting me in the village to ride the bus in with me. We weren't sure where to leave our cars for the day. We were catching the bus in front of Feather Hill and that is a private parking lot. A few people told us they park in the Municipal lot across the street. So we did and so did a couple of other cars that morning. We never noticed any signs. When we got back that evening both of our cars had been ticketed, but the car across from us that had parked that morning was not. So we looked for any signs. The one we thought was a sign for handicapped parking at the beginning of the lot was a 2 hour parking sign. We never knew it was a 2 hour lot. We pleaded not guilty and paid a lesser fee at the end of June. While we were there we ran into a number ofpeople who were ticketed in that lot too. They also weren't aware of the 2 hour limit. So here's my issue. There is no place to park in the village for more than 2 hours `-- without getting a ticket. I understand time limits on the street, but not in the lot. My suggestion is to make it a day lot. Then if someone is abusing it and leaving their car there for more than one day they could be ticketed. I'm hoping that you will look into this. Thank you. I wonder if the County agrees. I bring it up because the County recently repainted the intersection, but did no marking of the southern part of the circle and there is no signage at the spot where the circle meets Boisseau when you are trying to make a left onto Boisseau to get to Yennecott. That made me wonder if that part is considered the Town's responsibility. It seems there ought to be a sign there—I've never seen a circle intersection without a sign before. Each family has deeded beach rights to a beach located just west of the Mattituck town beach.To access this beach there is a private walkway located on Peconic Bay Boulevard on the west side of Marlene Lane.The families(which include many children) use this walkway to access the beach and must cross Peconic Bay Boulevard. The request in 2012 was for the Southold Town to consider installing stop signs at both the East and West side of Marlene Lane on Peconic Bay Boulevard.Cars routinely travel at dangerous speeds on this road and often do not stop when pedestrians, including children,cross at this intersection.As reported then and repeated now there have been a number of"close calls"crossing at this intersection and the visibility of seeing a speeding car is also difficult. Our request at that time was denied due to the lack of accident history. It is very disturbing that someone must be injured or killed before any action is taken.After that request was denied one of our members spoke directly to the Southold Town Supervisor,Scott Russell,who was very familiar with this intersection and seemed to support our request, but again noted a lack of accident history. During-that conversation Mr. Russell suggested that if stop signs do not get approved perhaps we could request a pedestrian crosswalk.A pedestrian crosswalk requires cars(per NY State traffic law)to stop at this crosswalk when pedestrians are present. At the June 14th meeting of the MLCA it was discussed again and unanimously voted to send in this letter to re-consider the stop signs or the pedestrian crosswalk. there is an "adverse statistic"created at this intersection. Supervision of Project: a. Roles and Responsibilities The Reasonable Local Official (RLO) shall be the Project Manager, James Richter, Town Engineer for the Town of Southold, unless defined otherwise in this document. The project will be staffed by Stephen Normandin, PE as Engineer-in-Charge(EIC) providing office engineering services and Ronald Patac as the full time field inspector, both from The RBA Group. The Town of Southold will provide an EIC who shall monitor the execution and progress of the project. He shall be responsible for ensuring conformance with the contract plans and specifications. He will develop and document estimates of completed work and prepare contractor payment requisitions. He shall monitor daily construction activities of the contractor for conformance with the contract plans and specifications; take measurements of all unit price items and compute areas and volumes of completed work; and monitor the execution of the project's maintenance and protection of traffic plan to assure the Contractor adheres to the contract requirements. Project records shall be maintained at Town of Southold Town Hall, 53095 Route 25, Southold, NY 11971. b. Consultant and Funding The EIC and Inspector will be from The RBA Group, an engineering consulting firm. Reimbursement for construction inspection shall be made at 80%federal, 20% local. C. Inspector Qualifications The EIC and Inspector shall have experience that is commensurate with the work involved including knowledge in construction as well as a familiarity with MURK procedures. A resume for both the EIC and Inspector is attached. d. Chain of Command The project staff hierarchy is illustrated in the attached organizational chart. The final determination for dispute resolution will rest with the Town of Southold. II. Contract and Project Requirements Letting Date: May 7, 2013 Contract Award Date: To be Determined Completion Date:Augu4,-U 2014 Prime Contractor: KJB Industries Award Amount:--$122,411.00 a. Disadvantaged Business Enterprise(DBE) Goal 1 This contract has a 0% DBE participation goal. b. Equal Employment Opportunity (EEO) Goals This project has a female participation goal of 6.9% and a minority participation goal of 5.8%. C. Permits There are no permits required for this project. d. Requirements of Railroads and Utility Companies There are no utility relocations anticipated for this project. The EIC will coordinate as necessary any utilities that need to be adjusted to the proposed grade with the respective utility companies and the Contractor if necessary. e. Commitments made to other municipalities, emergency service providers, schools, etc. The EIC and the Project Manager will be responsible for keeping the public, affected property owners, and key organizations apprised of road closure, parking restrictions and construction activity. Dates of road closures/parking restictions will be published and handed out to affected residents/commercial businesses. Affected schools, police, and fire departments, and ambulance companies, among other affected organizations will be notified by fax. The EIC will coordinate these notifications in close cooperation with the Contractor. f. Commitments contained in Design Approval Document that are to be fulfilled during construction No specific commitments were documented in the DAD. g. As-Built plans documentation procedures The EIC shall maintain a record set of plans that will document all modifications to the design plans, which will be considered As-Built plans and filed with the Town at the end of the project. III. Contract Administration a. Subcontractor Approval Process The Contractor does not intend to use any subcontractors. The EIC will remind contractors of the requirements of Form FHWA 1273.1.2 (contained in Appendix 1 of the Bid Proposal)which states that the contractors shall insert all of the stipulations of the contract into all contracts with the subcontractors and that,the prime contractor is responsible for the compliance of all subcontractors. Any subcontract found lacking these stipulations will not be eligible for federal reimbursement. b. Change Order Approval Process All Orders on contract must follow Section(s) 109-02 and 109-05 of the NY State Standard Specifications. All Order(s) on Contract should be submitted to NYSDOT Construction prior to completion of additional work by the contractor. The Order(s) on Contract should be submitted to NYSDOT for their review and approval. The EIC shall recommend approval to the Project Manager who will have final approval of all 2 Change Orders. After Sponsor review and before final approval, a copy of all Change Orders will be sent to the Regional Local Projects Liaison (RLPL). The RLPL's approval will be requested for any change order that: 1. adds significant new items, 2. changes the character of the work materially, in kind or nature from that included in the original proposed construction contract, 3. changes the quantity of any major item of work, fixed quantity item, or composite item (as defined in the contract) above 125% or below 75% of the original contract quantity, 4. proposes changes outside the contract limits, 5. proposes changes outside the contract scope, 6. the value of the proposed change order exceeds the contingent amount established in the State/Local Agreement C. Procedures to Ensure DBE Compliance In accordance with the DBE section of Appendices 12, the Contractor will make a good faith effort to utilize DBE's as his subcontractors. The Contractor will complete form AAP 19LL and AAP 20LL. The Sponsor and the Contractor will collaborate on all DBE reporting requirements. d. Procedures to Ensure EEO Compliance The EIC will interview the Contractor's EEO officer to insure that the Contractor is fully familiar with the Code of Federal Regulations, Title 41, Chapter 60, 60-1.1, Equal Opportunity Clause and 60-4, Construction Contractors, Affirmative Action Requirements; and Form FHWA 1273.111, Non-segregated Facilities; all of which are included in the Bid Proposal. The EIC will monitor the Contractor's Compliance with these requirements. The Sponsor and the Contractor will collaborate on EEO reporting requirements. The EIC will insure that the following forms are completed: The FHWA requires that the Prime contractor as well as all approved subcontractors should be registered and have access to the Equitable Business Office (EBO) system. The use of the EBO system is mandatory for all municipalities granted by Federal Aid money. For information and registration use the following location: dot.sm.mo.localprog rams.ebo. Monthly: AAP-33LL, Monthly Employment Utilization Report With each payment: AAP-21 LL, Contractor Report of Contract Payments e. Wage Rate Compliance Procedures Sponsors must comply with the following: • Collect and maintain all payroll records for 5 years after the completion of work. • Designate, in writing the individual responsible for collecting payrolls from the Prime Contractor. •The sponsor's designated employee's name must be posted in a conspicuous location at the project-site. New York State Department of Labor requires that all public projects greater than $250,000.00, laborers, workers, mechanics on site are required to be certified in the OSHA 10 hour Construction Safety & Health Class. The Sponsor must make the Prime Contractor's filing of payrolls with the Sponsor a condition of payment. If the Prime Contractor fails to file payrolls or will fully underpays wages, the 3 contractor can face possible felony charges. The EIC will audit the Contractor's Certified Payroll reports on a weekly basis. The EIC will also conduct wage rate interviews with at least Y2 of the Contractor's labor force that matches the Contractor's payroll frequency. The statement of compliance as required by Form FHWA 1273.V.2, (See PLAFAP 1273 Requirements) manual Statement and Payrolls—Payroll Records,will be strictly enforced. The EIC will compare the actual pay rates and supplemental benefits paid to the labor force with the State and Federal Wage rates published in the Bid Proposal. The contractor will be advised immediately in writing if there is a violation of this contract requirement, and will have one week to rectify the disparity. The EIC or Inspector should conduct wage rate interviews periodically (using MURK 10 form)with the workers in the field. f. Procedures to Ensure Worker Safety Prime contractor must submit to the EIC and the Sponsor, a Health &Safety Plan for review and approval. Sponsor must approve the Health and Safety Plan. A copy of the Health &Safety Plan must be submitted to the NYSDOT for review. The appointed Safety Officer shall be Bob Panchak, PE of KJB Industries; phone 631.727.4243. Keep NYSDOT informed as to the prime contractor's safety schedule. Include the schedule of safety meetings (whether it is weekly or monthly) along with any meeting minutes, as well as sign-in sheets as part of the project records/files. The EIC will have the Contractor's and Subcontractor's approved site specific safety plan on file prior to the start ofany work on site. The EIC will ensure that the contractor have on site at all times at least one person skilled in safety and health procedures and familiar with State and Federal safety and health regulations, whose responsibility it will be to monitor methods and procedures. This person will attend the preconstruction meeting, and will be approved by the EIC as qualified and will review all safety plans and procedures the Contractor will employ during the work. It is the Contractor's responsibility to only have on contract site those workers who successfully completed the OSHA 10-hour construction safety course and to have each of the worker's certificates of completion with the project records available for review by NYSDOT and FHWA. g. Claim and Dispute Resolution Procedures The Town purposes a three-step process to resolve contract claims or disputes. Initial notification of a claim or dispute will be required in writing from the contractor to the Project Manager. The Project Manager will work to resolve the claim/dispute. Should the Project Manager be unable to resolve the issue to the satisfaction of both parties, the Town of Southold's Town Supervisor will be asked to assist in achieving a resolution. The initial contact with the Supervisor's office should be made by the Project Manager but may be made by the contractor, in writing, if the Project Manager has been given the necessary initial written notification. The final step in this process will be to forward the claim/dispute to the Town's Attorney for resolution. Once again, the Project Manager should make contact with the Town Attorney's office. However, the contractor may contact the Town Attorney's office, in writing, if the Project Manager and the Supervisor's office have been notified in writing. h. Scheduling and Time Extensions As required under Section 108-01, Start and Progress of Work, the contractor will be required to 4 provide a work schedule within 5 days after the commencement of work. The EIC and the Project Manager will review this schedule and monitor the Contractor's compliance with this schedule. If the Contractor does not progress the work at the rate proposed, he will be advised in writing of failure to comply with his schedule and he will be ordered to provide an updated schedule indicting the means that he will use to guarantee the completion of the project on time. lve ,u-�( Completion date is set to be A-idgust,34, 2014. No extensions of time will be allowed unless specifically negotiated under Section 105-14, Dispute Resolution and Disputed Work Provisions. An extension of time request by the Contractor must be approved by the Town of Southold. Any time extension request must be submitted to NYSDOT Construction for their review and approval. i. Record Keeping and Record Retention Procedures The project will be administered and monitored using the NYSDOT Manual of Uniform Record Keeping (MURK). The EIC will complete daily logs and any special daily reports included in the NYSDOT Construction Inspection Manual (CIM), including structural concrete and asphalt paving reports. The Town of Southold anticipates utilizing an excel spreadsheet for documentation and pay estimate purposes. The EIC is responsible for the project Daily Contract Dairy sheets (Murk-2). The EIC shall maintain and sign these pre-numbered, loose-leaf type diary sheets which shall contain daily entries under his direction. All diary entries must be made in black ink. If the EIC is absent from the job, daily entries shall be made under the direction of, and signed by the person designated to be in charge. When inspection is by a Consultant, the EIC will prepare the diary and the Project Manager will review it, adding comments if needed, and countersign it. The Engineer's Daily Project Diary and the Inspector's Daily Report must be part of the project records. All records shall be maintained at the Town of Southold Town Hall, 53095 Route 25, Southold, NY, 11971. The MURK 2, the project daily diary, is the most important project document. It should be complete and understandable to anyone unfamiliar with the project.At the same time, it should be as brief as possible without sacrificing necessary information. The project Diary, along with Inspector's reports and other project records, shall be available for inspection by FHWA and NYSDOT at the engineer's project field office at all times. Upon completion of the project, all documents will be catalogued and copied. Original documents will be retained by the Sponsor. Close out materials requested by the RLPL will be provided. All documents will be retained for three years after the State/Local Project Agreement has been closed. IV. Construction Inspection a. Inspection Plan The EIC shall be responsible for ensuring conformance with the contract plans and specifications. He will develop and document estimates of completed work and prepare contractor payment requisitions. He shall monitor daily construction activities of the contractor for conformance with the contract plans and specifications; take measurements of all unit price 5 items and compute areas and volumes of completed work; and monitor the execution of the project's maintenance and protection of traffic plan to assure the contractor adheres to the contract requirements. The prime contractor is responsible for following proper Maintenance and Protection of Traffic (M&PT) as per contract plans, National MUTCD with NY State supplement or NY State standard sheets within the contract job limits. In the event the prime contractor and/or subcontractor/utility companies are working on the jobsite without proper M&PT, the Sponsor/Sponsor's representative shall be responsible for charging the prime contractor for Basic Work Zone Traffic Control non-payment, following the amount on Table 619-6 of the NY State Standard Specifications. b. Quality Control/Quality Assurance Plan The Sponsor will make periodic visits to the jobsite to ensure that work is being completed in accordance with the contract, including the specifications. The EIC will be interviewed prior to the start of the project to insure that she is fully familiar with the inspection procedure for any item of work in the project, prior to start of work on that item. Quality Assurance and Quality Control shall be maintained trough the implementation of the NYSDOT Material Inspection Manual. NYSDOT CIM will be referenced for that assurance. The EIC shall take the concrete test cylinders, tag them, prepare appropriate paperwork and transfer them to a NYSDOT approved laboratory. All cost for testing to be paid by the Town of Southold. C. Material Testing Procedures The Sponsor is responsible for material testing and acceptance. These duties can not be assigned or delegated to the contractor. The material testing procedures must be tailored to each project. The field Inspector must be ACI certified in order to perform concrete testing (slump tests, air tests and taking cylinders.). The Sponsor must hire the materials testing company. If the Inspector is not ACI certified, the Town shall supply an ACI inspector to be on site during the days of concrete placement by contracting with a NYSDOT certified material testing laboratory and ACI inspector. Two (2) concrete test cylinders shall be taken per day. Cylinders shall be tested for compression strength by a NYSDOT approved laboratory. One(1) cylinder shall be saved for testing at 45 days, if the 28 day test on the initial cylinder proves unsatisfactory. Soil Mechanics Drilling Corp. will serve as the independent laboratory for testing (Contact Carl Vernick, Phone 516.221.2333). d. Shop Drawing Approval Process Shop drawing review will be the responsibility of the design engineer through the EIC. The review time will be as allowed by the Specifications and/or Contract Plans for the subject item. V. Coordination with Others a. Anticipated efforts to ensure compliance with permits The EIC will notify all permitting agencies of the schedule of each project. No work will be progressed prior to the full execution of any permit. b. Anticipated efforts to coordinate,with affected utility companies and railroads The EIC will notify the appropriate utilities during construction to coordinate their adjustments. 6 C. Anticipated efforts regarding coordination with other municipalities, emergency service providers, schools, etc. The EIC and the Project Manager will be responsible for keeping the public, affected property owners, and key organizations apprised of road closure, parking restrictions and construction activity. Dates of road closures/parking restictions will be published and handed out to affected residents/commercial businesses. Affected schools, police, and fire departments, and ambulance companies, among other affected organizations will be notified by fax. The EIC will coordinate these notifications in close cooperation with the Contractor. VI. Close-Out a. Contract Acceptance and Close-out Procedures The project close-out will be the responsibility of the Project Manager. The project will be inspected by the EIC and the Project Manager for completeness. If the project is deemed to meet the contract requirements, the sponsor will notify the Regional Local Projects Construction Monitor that a final inspection can be performed. After approval by all parties, the Final Payment will be completed by the contractor, submitted to the EIC so that final payment can be made. The Regional Local Projects Construction Monitor will be notified and the sponsor will provide all required close out information so that the State/Local Agreement can be closed.
