This list is subject to monthly revision, and updated lists will be mailed monthly to all FT-943 and FT-945 filers. Anyone who purchases from a reseller and is given a copy of an Authorization to Issue Preliminary Certifications should verify that the supplier's name is on this list. If it is not, or if you fail to verify that the name is on the list, you will be considered not to have accepted in good faith the Authorization to Issue Preliminary Certifications. If your supplier presents an Authorization to Issue Preliminary Certification but is not on the list, you should write to the Fuel Tax Unit, Building #9, Room 402A, W. A. Harriman Campus, Albany, New York 12227, to determine if the supplier has been authorized to issue Preliminary Certifications since the publication date of the list. WAREX PETROLEUM NOTICE Your supplier need not appear on this list if', at the time of delivery of the motor fuel to you, he gives you either: 1. a properly completed Certificate of Prepayment of Sales Tax and Payment of Motor Fuel Tax (Form FT-935); or 2. an invoice or other billing document that contains the following: -seller's name and address; -seller's Sales Tax Identification Number and Motor Fuel Registration Number, if any; -purchaser's name and address; -type of fuel purchased (leaded, unleaded, premium); -a separately stated amount per gallon for each tax included in the selling price; and -a signed statement that the amounts of tax shown have been or will be paid by the seller or his supplier. If the seller is a registered motor fel distributor under Article 12-A, no signature will be required. If the payer of the Motor Fuel Tax is other than the seller, that payer must be named. If your supplier does not provide either of these documents, your supplier's name must appear on this list The supplier must also furnish you with a copy of his Authorization to Issue Preliminary Certification of Prepayment. of Sales Tax and Payment of Motor Fuel Tax (Form FT-952), valid as of the date of your delivery. An invoice or other billing document given at the time of delivery which contains the following , will be acceptable as certification of prepaid tax: -Seller ' s name and address Seller ' s Sales Tax Identification Number and Motor Fuel Registration Number, if any -Purchaser ' s name and address -Type of fuel purchased ( leaded, unleaded, premium) -A separately stated amount per gallon for each tax included in the selling price -A signed statement that the amounts of tax shown have been or will be paid by the seller or his supplier. The Police Department maintains a 5,000 gallon capacity tank at the Police Headquarters at Peconic, New York, and uses approximately 55,000 gallons of unleaded gasoline annually. The Highway Department maintains a 5,000 gallon tank for regular gasoline and a 2,000 gallon tank for unleaded gasoline. All tanks are located at the Highway Department premises at Peconic Lane, Peconic, New York. The Highway Department uses approximately 30,000 gallons of regular gasolin annually, and approximately 2,200 gallons of unleaded gasoline annually. In submitting bids, the bidder may either submit a fixed price per gallon to be charged throughout the term of the contract, or a price which is based on a rack price at the bidder's source of supply. If a rack price is used, the lowest rack price within five (5) days of each delivery to the Town must be used. Verification of the rack price used by the bidder must be supplied by the bidder to the Town upon request. The successful bidder will be required to make deliveries to the Town within eight (8) hours of a request for a delivery from any Town Official, and such bidder will be required to make deliveries on any day of the week, including Saturdays, Sundays, and Holidays. The Police Department maintains a 5,000 gallon capacity tank at the Police Headquarters at Peconic, New York, and uses approximately 55,000 gallons of unleaded gasoline annually. The Highway Department maintains a 5,000 gallon tank for regular gasoline and a 2,000 gallon tank for unleaded gasoline. All tanks are located at the Highway Department premises at Peconic Lane, Peconic, New York. The Highway Department uses approximately 30,000 gallons of regular gasoline annually, and approximately 2,200 gallons of unleaded gasoline annually. In submitting bids, the bidder may either submit a fixed price per gallon to be charged throughout the term of the contract, or a price which is based on a rack price at the bidder's source of supply. If a rack price is used, the lowest rack price within five (5) days of each delivery to the Town must be used. Verification of the rack price used by the bidder must be supplied by the bidder to the Town upon request. The successful bidder will be required to make deliveries to the Town within eight (8) hours of a request for a delivery from any Town Official, and such bidder will be required to make deliveries on any day of the week, including Saturdays, Sundays, and Holidays. The successful bidder must make deliveries in such quantities of 250 gallons or more, as may be required. Bid prices are not to include any Federal, State or Local Tax from which the Town of Southold is exempt.
