2. Final Approval from Health Dept,of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.

of Code Compliance from architect or engineer responsible for the building.

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 3
sets of plans,accurate plot plan to scale. Fee according to schedule.

Description of work to be performed:
Instal a freestanding, wood burning stove (Jotul F400), in an existing masonry fireplace. the
stove pipe will be vented into the existing fireplace chimney as specified in the owners manual
(excerpt included below)
3.2 Hearthmount into a Masonry a
Fireplace
The Jvtul f 400 may be installed into a masonry fireplace
provided the height of the opening is a minimum of zg 1/2Use
of the Short leg Package will reduce the stove height by z 1/4"
(57 mm). Connector extends
to first flue tile
Building code requiresthat thefireplace damper plate be removed
or secu rely fixed in the open position.A connector pipe must they
extend from the stove's flue exit through the dam per area of the
fireplace and into the chimney tile liner.See figure 6. Damper opening is

The inside area of theflue liner must not be less than the area of _i plate and sealant
the stove flue collarand cannot be more than threetimes greater
than the cross sectional area of the stove flue collar.
If the chimney liner is too large to accommodate the stove,an
approved relining system must be installed to resize the flue. pp r-
A new sheet metal damper block off plate must be installed
around the connector pipe at the damperframe and sealed with
the proper sealant(usually High Temp Silicone).
