The kind and amounts of insurance are as follows: a) WORKER"S COMPENSATION INSURANCE: As required by New York law. b) MOTOR VEHICLE LIABILITY INSURANCE: Each policy shall cover the contractor and the Town of Southold with a combined single limit of not less than $1,0009000. c) OWNER AND CONTRACTOR PROTECTIVE LIABILITY POLICY: Naming the Town of Southold as the party being insured, in an amount of $1,000,000. 1 13.0 BID SECURITY: Bid security shall be submitted with the bid in the amount I of five (5%) percent of the bid. Security shall be in the form of a certified check or Bid Bond by a Surety Company authorized to do business in the State of New York. The bid security, except that of the successful bidder, will be returned as soon as practicable after the bid openings. 14.0 PERFORMANCE BOND: The contractor shall furnish Performance and Payment Bonds as security for the faithful performance and payment of all his _ obligations under the contract. These bonds shall be in amounts at least equal to the contract price, and in such form and with such sureties as are licensed to conduct business in,New York State and are named 'in the current list of "Surety Companies Acceptable on Federal Bonds" as published in the Federal Register by the Audit Staff Bureau of Accounts, U.S. Treasury Department. 15.0 NON-COLLUSIVE BID CERTIFICATION: All bidders shall execute and attach to Their bids an affidavit substantially in the form herein provided to the effect that he has not colluded with any other person, firm, or corporation in regard to any bid submitted. 16.0 OTHER LAWS: In all operations related to the work, all other State Laws or Federal Laws, local ordinance and laws, controlling or limiting in any way the actions of those engaged in the work (including their wages, hours and.benefits) , shall be strictly complied with by the Contractor and all employees working under his direction. The rate shall include all machine, labor, supplies, insurance, mobilization and demobilization costs, and any other costs associated with the project. All contract shredding work under this project will be performed at this site. 3.0 MATERIAL: The Town makes no guarantee or representation regarding the size, quantity, or consistency of debris to be processed. There is approximately 50,000 cubic yards of material consisting mostly of small brush, limbs, leaves, and logs of varying lengths. The material has been pushed together tightly by payloader and bulldozer. There are occasional tree stumps and logs of larger diameter (greater than 12") mixed with the material. THE TOWN MAY CHOOSE TO HAVE ONLY MATERIAL OF 12" DIAMETER OR LESS PROCESSED BY THE CONTRACTOR (see Section 18.0 "OPTIONS" below) . For this reason, the material should be inspected at the site by interested bidders prior to the bid. 4.0 CONTRACTOR REQUIREMENTS: The Contractor shall provide one mobile, high capacity chipping or shredding machine to accept the material specified. The contractor shall provide the personnel to. operate, maintain, and repair the equipment. The contractor shall provide all necessary fuel and supplies. The contractor will provide specified services five days per week (Monday through Friday) between the hours of 8:00 AM and 4:00 PM. 5.0 TOWN ASSISTANCE: The Town will move debris to the shredder and will remove the shredded material. Removal will be accomplished through the use of a Town-owned truck which will be either loaded directly from the shredder's conveyor belt or by a Town-operated payloader. 'The Town will prepare the working location to meet the contractor's needs. If the Town chooses to have the contractor process only material of 12" or less in diameter, the Town will assist the contractor in separating out the larger material. The Town will be responsible for disposing of any material which, under the terms of this contract, cannot be processed. The Town will not load the chipper/shredder. 6.0 FINISHED PRODUCT: The contractor may produce a shred of varying length but that is in any case not greater than 2" in diameter. 7.0 TERM: The term of the contract will be from the date of execution through July 31, 1992, by which time the work must be completed. If work is not completed by that date, the Town reserves the right to extend the contract at the agreed upon rate until the processing of said material is completed. Terr Southold Town Clerk May 6, 1992 TOWN OF SOUTHOLD ^ h BID SPECIFICATIONS F4R THE PROCESSING OF STORM DEBRIS 1.0 INTENT: It is the intent of these specifications to describe the labor and equipment services needed for the volume reduction by shredding or chipping of brush debris generated within the Town of Southold by Hurricane Bob. 2.0 SITE: The debris is stockpiled in the sand borrow pit at the Southold Town Landfill, County Road 48, Cutchogue, Suffolk County, Long Island, New York. All contract shredding work under this project will be performed at this site. 3.0 MATERIAL: The Town makes no guarantee or representation regarding the size, quantity, or consistency of debris to be processed. There is approximately 50,000 cubic yards of material consisting mostly of small brush, limbs, leaves, and logs of varying lengths. The material has been pushed together tightly by payloader and bulldozer. There are occasional tree stumps and logs of larger diameter (greater than 12") mixed with the material. THE TOWN MAY CHOOSE TO HAVE ONLY MATERIAL OF 12" DIAMETER OR LESS PROCESSED BY THE CONTRACTOR (see Section 18.0 "OPTIONS" below) . For this reason, the material should be inspected at the site by interested bidders prior to the bid. 4.0 CONTRACTOR REQUIREMENTS: The Contractor shall provide one mobile, high capacity chipping or shredding machine to accept the material specified. The contractor shall provide the personnel to operate, maintain, and repair the equipment. The contractor shall provide all necessary fuel and supplies. The contractor will provide specified services five days per week (Monday through Friday) between the hours of 8:00 AM and 4:00 PM. 5.0 TOWN ASSISTANCE: The Town will move debris to the shredder and will remove the shredded material. Removal will be accomplished through the use of a Town-owned truck which will be either loaded directly from the shredder's conveyor belt or by a Town-operated payloader. .The Town will prepare the working location to meet the contractor's needs. If the Town chooses to have the contractor process only material of 12" or less in diameter, the Town will assist the contractor in separating out the larger material. The Town will be responsible for disposing of any material which, under the terms of this contract, cannot be processed. The Town will not load the chipper/shredder. 6.0 FINISHED PRODUCT: The contractor may produce a shred of varying length but that is in any case not greater than 2" in diameter. 7.0 TERM: The term of the contract will be from the date of execution through July 31, 1992, by which time the work must be completed. If work is not completed by that date, the Town reserves the right to extend the contract at the agreed upon rate until the processing of said material is completed. The Town requests the services as soon as possible. Time is of the essence, and will be a factor in the award of the contract. 8.0 RECORD: Lt is anticipated that the Town will provide a 10-cubic yard capacity Mack dump truck for removing processed material from the shredding area. As each truckload of processed material is removed, the contractor and the Town truck operator will fix their initials to a chit verifying such removal. In the event that not all processed material is removed by truck by the end of each day, the contractor and the site supervisor will measure and agree upon the amount of such material remaining on the ground next to the shredder, still to be moved.
