Dear Mr.Richter:
As requested by the Southold Town Engineering Department and based upon the construction schedule
provided by Steve M. Maki, Inc. on May 19, 2014, L.K. McLean Associates, P.C. (LKMA) has
prepared this proposal for the Town of Southold for additional construction administration and part-time
construction monitoring Sam= associated with the rehabilitation of the dips. Our original proposal
dated May 21,2013 included 20 hours of inspection and 10 hours of contract administration services for a
6 week time period.(watch all four farms concurrently). LKMA has utilized an inspector with a lower
hourly rate in order to stretch the;time period well beyond the initial 6 week construction period. As of
May 21,2014 LKMA has 37 hours of construction monitoring services left Oust under two weeks)in the
initial budget.
Based:upon the construction schedule f idshed by the Contractor it appom that our sery oess will meted to
extend at least to the end of June 2014. It is likely that there will be contract closeout responsibilities and
a limited amount of contract;administration services that extend into the first couple of weeks in July.
Based upon these,above refere coed conditions we estimate performing the fo)lowing additional.tasks;
• Provide additional construction inspection and contract administration services through the end of
June and intro the first two werlcs of July 2014. We estimate an additional 75 hours of
construction ikon services($70/hour)and 30 thirty hours of eos"ot administration services
(S1251hour).$9,00000

To amend the original agreement to increase the estimated total construction cost due a differing
site condition, not previousely identified, requiring an additional cost to remove unsuitable
material and rebuilding this newly discovered area.
Estimated total cost of construction for DSR S-TSH-01 [paragraph Il and III(2)(b)],NRCS
Technical Assistance [paragraph III(2)(a.) (v.)],NRCS Financial Assistance [paragraph
III.(2)(b)(i)], and Sponsor cost-share [paragraph III.(2)(b)(ii.)] are herey revised as follows:
Total Estimated Cost of Construction for S-TSH-01: $2,178,895.50
Sponsor Share FA: $544,723.87 (increase of$12,499.37)
NRCS Share FA: $1,634,171.63 (increase of$37,498.13)

RE: Cooperative Agreement#68-2C31-3-0007 w/Town of Southold
Project: DSR-S-TSH-01, Dike Repair
Dear Ms. Eipp:
In accordance with Section III(1) of the above-referenced Agreement, the Town
of Southold hereby requests an extension of the Project Period for the pending
Emergency Watershed Protection Project for an additional 220 days beyond the current
completion date of July 20, 2014.
As you may be aware, the heavy snowfall and rainfall experienced on the North
Fork of Long Island this previous Winter and Spring have hindered the repair of the
dikes. Furthermore, these delays have pushed repair activities into the hot and dry
.summer months which lie outside of the established planting windows for this Project.
Therefore, while construction activities will be completed this summer, the dikes will not
be planted until September. This extension is requested so that the Town can remain
compliant with its responsibilities and obligations under the Agreement.
Kindly contact my office upon receipt of this correspondence to confirm NRCS's
consent to amend the Cooperative Agreement accordingly. Thank you for your
assistance.
