A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3-. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1 % lead. .
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliahce-from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in pail for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the wort: authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Building Zone Ordinance of the Town of Southold, Suffolk County. New York. and other applicable Laws, Ordinances or

Storm Water management
General Property Conditions:
1. The residence was built in the 1950's, and the previous owner obtained a Certificate
of Occupancy in 1985.
2. We will excavate to pour 312 sq/ft of new foundation. There will be no additional fill
brought in; the only material to be removed will be that which results from the new
foundation and 2 drywells.
3. This lot is a mature residential property with no history of drainage problems,
pooling or storm water run-off onto neighboring properties or Bay Avenue.
4. The topography is flat, with a slight westerly pitch away from the house.
5. There are no potential surface waters of the State of New York and/or MS4 that will
be impacted by construction.
During Construction:
1. The construction access road will be the existing RCA driveway off of Bay Avenue.
This is an old, well-compacted driveway that will handle the necessary construction
traffic without spilling mud or other construction debris onto Bay Ave. In the
unlikely event that the driveway should become worn from construction traffic, or
should mud or debris spill into the road, we will add RCA to the driveway and the
driveway entrance where it meets Bay Ave. It is our intention to maintain the
driveway for the homeowners' future use once the house is completed, so we will
take every precaution to minimize damage to the driveway during construction.
2. There is no need for silt fencing or hay bales as the property is flat and has always
contained the storm water.
3. On-site storage of construction material shall be limited to material necessary to re-
build the addition using conventional building products. All waste storage shall be
contained in a single 30 yard dumpster, to be promptly emptied when full.
4. During construction, the contractor, New England Barns and Fine Homes Inc.
represented by William Gorman, shall be responsible for daily inspections and the
repairing, replacing and maintenance of the Erosion and Sediment Control Practices.
Post-Construction:
1. We will add two (2) drywells, 8' diameter by 4' deep, at the locations indicated on
the survey. The roof gutters will be piped directly into the drywells to manage the
storm water and minimize erosion.
Roof area: 1323 sq/ft.
Drywell capacity: 8' diameter, 4' deep= 1000 sq/ft of roof area run-off
during a 2" storm water event.
2. After construction, we will add additional RCA to the existing driveway (construction
access road) and compact that material to alleviate future material loss from the
driveway onto Bay Avenue.
3. We will re-plant grass throughout the lawn as needed, and we will plant shrubs and
bushes around the perimeter of the house.

INSTITUTE (DHI) GUIDELINES FOR SCHEDULING. FURNISH INITIAL DRAFT OF SCHEDULE AT THE EARLIEST POSSIBLE DATE, IN ORDER TO FACILITATE THE FABRICATION OF OTHER WORK. FURNISH
FINAL SCHEDULE AFTER SAMPLES HAVE BEEN APPROVED.
2. PRIOR TO SUBMITTAL OF THE FINAL HARDWARE SCHEDULE AND DELIVERY OF HARDWARE,
SUBMIT ONE (1) SAMPLE OF EACH EXPOSED HARDWARE UNIT. SAMPLE WILL BE REVIEWED BY
THE ARCHITECT FOR DESIGN, COLOR, AND TEXTURE ONLY. COMPLIANCE WITH OTHER
REQUIREMENTS IS THE EXCLUSIVE RESPONSIBILITY OF THE CONTRACTOR. SAMPLES APPROVED BY THE ARCHITECT SHALL BE TURNED OVER TO THE OWNER FOR ATTIC STOCK.
3. DO NOT PROCEED WITH INSTALLATION OF HARDWARE UNTIL SAMPLES HAVE BEEN APPROVED.
APPROVED SAMPLES MAY BE INSTALLED IN THE WORK AFTER SUBSTANTIAL COMPLETION OF WORK.
4. PROVIDE KEY SCHEDULE SHOWING ALL KEY NUMBERS AND SPACES TO WHICH EACH PERMITS
ENTRY SHALL BE PROVIDED. CONSULT WITH THE OWNER, AND PROVIDE NECESSARY ASSISTANCE
REQUIRED FOR DEVELOPMENT OF THE SCHEDULE BEFORE SUBMITTING FINAL KEY SCHEDULE. AFTER FINAL APPROVAL HAS BEEN RECEIVED, THE SCHEDULE ALONG WITH THE KEY GATHERING
ENVELOPES CONTAINING KEYS FOR EACH LOCK, ENDORSED WITH LOCK NUMBER AND SPACE
DESIGNATIONS, SHALL BE TURNED OVER TO THE OWNER.
5. THE HARDWARE PER THE MANUFACTURERS SHALL PROVIDE FULL REPLACEMENT WARRANTIES, AS
PER THE LIST BELOW. REPLACEMENT WARRANTIES SHALL NOT INCLUDE ANY LABOR COST.
