This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.

Dear Inspector Verity:
This letter shall serve to certify that the enclosed plans and specifications outlining the
finished second floor which was finished with gypsum wall board over ten years ago is in
conformance with the Building Code in effect at the time. The original house and second
floor was constructed to my knowledge over 100 years ago. The second floor has prior
certificates of completion for a bedroom and a bathroom. This application is to legalize the
rest of the second floor insulated and finished without a Building Permit. All roof and wall
framing cavities have been insulated to the full depth of cavity with insulation exceeding R
3.0 / inch. The plans comply with the provisions of the NYSRC Appendix J104 Alteration
Level 2 which requires that the Energy Construction Code conformance can be achieved
by NYSRC N1101.3.1. N1101.3.1 Exception 3 permits compliance without the requirement
of Rescheck Computations provided that all wall and roof framing cavities of an existing
structure (built prior to the 2000 Code) are insulated to the full depth of all framing
cavities with an insulation such as fiberglass batts having an R Value of 3 or more per inch.
This letter shall certify that I inspected the existing finished second floor of the subject
structure and it is in conformance with the NYS Uniform Building and Fire Prevention
Code.

d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 month?; from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.
