THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION NO. 2013-828 WAS
ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON
NOVEMBER 19,2013:
RESOLVED that the Town Board of the Town of Southold hereby authorizes and directs
Supervisor Scott A. Russell to execute the renewal Agreement between the Town of Southold
and Cornell Cooperative Extension in connection with the Eat Smart New York Program for
2013 wherein Cornell will conduct a six week series of educational workshops on food and
nutrition to seniors at the Human Resource Center during November and December 2013, at no
cost to the Town, subject to the approval of the Town Attorney.

FY 2013 Agreement between Cornell Cooperative Extension, Eat Smart New York and the
Mattituck Senior Nutrition Center
Extension Educator Maryann Birmingham, will teach a series of food & nutrition lessons at the
Nutrition Center located at: 750 Pacific Ave., Mattituck, NY 11952. The series of workshops will
begin on November 6, 2013 and on December 11, 2013. The group will meet from 11:00 am -
12:00 pm on Wednesdays.
The Nutrition Center Agrees to:
• Notify Maryann Birmingham prior to a scheduled lesson if unable to hold a session and to reschedule at
that time.
• Provide names and ages of participants to the educator.
• Provide staff, volunteers or helpers; who will assist the Community Nutrition Educator in collecting data
and conducting sessions. The names of those assigned to assist will be provided to Cornell Cooperative
Extension.
• Set up for the sessions prior to the time the Community Nutrition Educator arrives.
Cornell Cooperative Extension agrees to:
• Notify the Nutrition Center prior to a scheduled lesson, if unable to hold a session and to reschedule at
that time.
• Supply training materials, certificates, and other materials as well as some food items needed
for hands-on experience-
