Dear Mr. and Mrs. Koff:
The following action was taken at the Southold Town Board of Trustees regular meeting
held on Wednesday, February 15, 2023:
RESOLVED, that the Southold Town Board of Trustees APPROVE the request for a
Transfer of Wetland Permit #1684 from John Bertani to Howard & Lisa Koff, as issued
on September 28, 1983 and Amended on April 24, 1986; and
BE IT FURTHER RESOLVED, that the Southold Town Board of Trustees APPROVE
the Administrative Amendment to Wetland Permit#1684 for the as-built "T"
configuration of the 3'x13'8" ramp and 5'11"x20' float, in lieu of the previously approved
"I" configuration; and as indicated on the project plan prepared by Howard Koff,
received on January 30, 2023, and stamped approved on February 15, 2023.
Any other activity within 100' of the wetland boundary requires a permit from this office.
This is not a determination from any other agency.

Koff from John and Linda Bertani(Prior Owners of 1380 Oakwood Drive)
Dear Sirs:
We are the owners of the property located at 1380 Oakwood Drive,having purchased it '
from John and Linda Bertani on August 21, 2020. We understand that the dock permit for
the dock on our property is still in the name of the Bertanis. We are writing now to
request the consent of the Board of Trustees to transfer the dock permit from John and
Linda Bertani to us,Howard and Lisa Koff. In connection with our request, we grant the
Trustees permission to enter our property to view the dock.
In addition to our request for consent, we are seeking an amendment to the dock permit.
The dock is, and has been since we purchased it, configured in a"T" shape, while we
understand that the existing permit contemplated an"I" dock. We would like to keep the
dock as is, and believe this is in the best interests of all of our neighbors and users of
Goose Creek in that the T-configuration is less obtrusive than would be a dock that went
straight out into the creek.
Enclosed is a check for$150 (representing the application fees for the transfer and the
permit modification), along with two copies of a drawing of the dock, showing the
measurements of each section of the dock. Please note that the dimensions of each
section of the dock are the same dimensions as those specified in the existing dock
permit.

on the environment for the following reasons:
An environmental assessment has been submitted which indicated
that no significant adverse effect to the environment are likely to
occur should the project be implemented as planned.
Because there has been a response from the Board of Southold
Town Trustees and Southold Town Conservation Advisory Council indicating
that this project would not have a significant effect to the environment.
Because there has been no response in the allotted time from the
New York State Department of Environmental Conservation and the Southold
Town Building Department, it is assumed that there are no objections nor
comments from those agencies.

which extends offshore from the O. H. W. M.

RESOLVED to recommend to the Southold Town Board approval
of the Wetland Application ~158 submitted by En-Consultants, Inc.,
on behalf of John Bertani to construct a bulkhead and dock in
Goose Creek, with the provision that the North end of the bulk-
head is placed back 5' and that the dock is placed 40' from the
bulkhead.

A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Clerk in quadruplicate, accompanied by writter~ permission from the owner of the property Jf not
the same as the applicant.
B~ Pilot Plan must be drawn to scale on the reverse ~ide of this application showing the location
of property, the names and locations of owners of adjoining properties, and a detailed description
of proposea project. (Note: Copies of the Pilot Plan may ye made by a copy machine and attached
to the Permit Application.)
C. No operation shall be initiated 3y the applicant until all permits that are required are issued.
D_ A detailea statement of the rehabilitation and proposed condition of the premises after the
work is completed must be given, including a survey if required.
E. If permit is being sought for o more sizeable excavation than the construction of a private
stogie dock or jetty, th'eh this appJication must be accompanied by a survey end topographical map
certified by a registered land survey or Professional Engineer. The horizontal control of survey
shalJ be based uvor, an approval local coordinate system. The vertica control for elevation and sound-
ing~shalt.be based upon United States Coast and Geodetic Survey and/or United States Geological
Survey datum.
F. A s~atement must accompany the application dqscribing any known prior operations conducted
on me premises in question and whether any prior licenses or permits have been issue~ to erect
structures or to dredge or aeposit fill on said arem~ses and whether any such permits or licenses
were ever revoked or suspende~ by a governmental agency.
G. A filing fee of $25.00 shall accompany the application. This filing fee includes one inspection.
