This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work,
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f Every building permit shall expire,if the work authorized has not commenced within 12 months after the date of
issuance or has hot been completed yvithiA 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

CONTRACTOR CERTIFICATION STATEMENT
Prior to the commencement of construction activity, the owner or operator must identify ALL contractor(s) and
sub-contractor(s) that will be responsible for installing, constructing, repairing, replacing, inspecting and
maintaining the erosion and sediment control practices included In the SWPPP; and the contractor(s) and
sub-contractor(s) that will be responsible for constructing the post-construction stormwater management
practices included in the SWPPP.
The owner and/or operator shall have each of the contractors and/or subcontractors Identify at least one person
from their company that will be responsible for implementation of the SWPPP. This person shall be known as the
trained contractor. The owner and/or operator shall ensure that at least one trained contractor is on site on a
daily basis when soil disturbance activities are being performed.
The owner and/or operator shall have each of the contractors and subcontractors identified above sign a copy of
the following certification statement below before they commence any construction activity:

"I hereby certify that I understand and agree to comply with the terms and conditions of the SWPPP and agree to
implement any corrective actions identified by the qualified inspector during a site inspection. I also understand j
that the owner or operator must comply with the terms and conditions of the most current version of the New

Storm Water management
General Property Conditions:
1. The residence was built in 2001 and was lost to fire in February of 2012. It is our
intention to re-build the house on the exact footprint by using the existing
foundation. We will not excavate except to repair and waterproof a small section of
the foundation. There will be no additional fill brought in, nor will any fill be
removed.
2. This 1.55 acre corner lot is a mature residential property with no history of drainage
problems, pooling or storm water run-off onto either Major's Pond Path or Rowe
Drive.
3. The topography is flat (please see photos P-1, P-2 and P-3), with a slight swale
running north/south in the southern area between the house and Rowe Drive.
4. The water table is located @ 12'-9", as indicated on the survey (S-2). This survey
was previously used to obtain approval from the Suffolk County Dept. of Health for a
sanitary system.
5. There are no potential surface waters of the State of New York and/or MS4 that will
be impacted by construction.
During Construction:
1. The construction access road will be the existing RCA driveway off of Major's Pond
Path (please see photo #'s D-1 and D-2). This is an eleven year old, well-compacted
driveway that will handle the necessary construction traffic without spilling mud or
other construction debris onto Major's Pond Path. In the unlikely event that the
driveway should become worn from construction traffic, or should mud or debris
spill into the road, we will add RCA to the driveway and the driveway entrance
where it meets Major's Pond Path. It is our intention to maintain the driveway for
the homeowners' future use once the house is completed, so we will take every
precaution to minimize damage to the driveway and apron during construction.
2. There is no need for silt fencing or hay bales as the property is flat and has always
contained the storm water.
3. On-site storage of construction material shall be limited to material necessary to re-
build the house using conventional building products. All waste storage shall be
contained in a single 30 yard dumpster, to be promptly emptied when full.
4. During construction, the contractor, New England Barns Inc. represented by William
Gorman, shall be responsible for daily inspections and the repairing, replacing and
maintenance of the Erosion and Sediment Control Practices.
Post-Construction:
1. We will add three (3) drywells, 8' diameter by 4' deep, at the locations indicated on
the survey (S-1). The roof gutters will be piped directly into the drywells to manage
the storm water and minimize erosion. The water table (12'-9") will accommodate
the drywells as dimensioned above.
Roof area: 2600 sq/ft.
Drywell capacity: 8' diameter, 4' deep= 1000 sq/ft of roof area run-off
during a 2" storm water event.
2. After construction, we will add additional RCA to the existing driveway (construction
access road) and compact that material to alleviate future material loss from the
driveway onto Major's Pond Path.
3. We will re-plant grass throughout the lawn as needed, and we will plant shrubs and
bushes around the perimeter of the house.
