A. For new building or new use:
I . Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:

Dear Mr. Verity;
Please be advised that with regazd to the above referenced premises, the Owner has completed the storm
related repairs to the three (3) first floor hotel units and lobby azea which were anon-structural cosmetic
restoration. In addition, minor alterations and repairs as shown on my drawing R-2 which accompanied
the application for the permit have been substantially completed.
Based on personal observation during the restoration, to the best of my knowledge and belief it is my
opinion that the work completed substantially meets or exceeds the requirements of the New York State
Building Construction Code.

INSTRUCTIONS
a This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fce according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every buildmg permit shall expire if the work authorized has not commenced within 12 motrdts after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a sew permit shall be required.

The following action was taken by the Southold Town Board of Trustees at their Regular
Meeting held on Wednesday, December 12, 2012:
RESOLVED, that the Southold Town Board of Trustees APPROVE the Administrative
Amendment to Wetland Permit #7770 to substitute for previously approved concrete
sidewalks, steps and landings for wood constructed walkways, landings and ramps
elevated to finished floor elevation and add stepped terracing of loose stone on filter
cloth down to existing grade; and as depicted on the revised project plan prepared by
Garrett Strang, received on December 4, 2012 and stamped approved on December
12, 2012.
Any other activity within 100' of the wetland boundary requires a permit from this office.
This is not a determination from any other agency.

Enclosed are four (4) copies of drawing R-1 to replace the copies of drawing R-1 previously submitted at
the time the application for storm related repairs at the above referenced premises was filed. These
drawings aze now in addition to drawing R-2 filed eazlier this week and aze relative to the fish mazket azea

Partial removal and replacement of flood damaged lower wall
cavity insulation to have an r-value equal to or grater than that
which is being replaced.
In addition, the following repairs were required when discovered at the time storm repairs were underway.
Refraining of the East wall and adjacent floor azea of the building from the South side of the fish
market to the North comer of the Studio hotel units and along the North wall of the northerly
studio units for a distance of approximately 10 feet due to extensive rot found in the structure.
Replacement of the Easterly interior partitions of the 3 studio units due to rot and necessitated by
relocating plumbing off the exterior wall to prevent freezing in the winter months.
Refraining of the demising wall between the fish market and the southerly studio unit due to
extensive rot in the structure.

Furthermore; the following alterations aze being undertaken in these azeas;
Installation of replacement windows and exterior doors at the studio units with proper size
headers to replace existing.
6" Build out of walls between studio units for sound attenuation of wall mounted TV's.
Installation of replacement and new windows and two new exterior doors at the fish mazket with
proper sized headers to replace existing.
Relocation of the existing shared toilet room at the fish mazket with associated plumbing work so
as to be off the exterior wall to prevent freezing during the winter. Please note this toilet room has
been redesigned so as to be accessible as per code.
Installation of a partition and new door in the hallway created by the relocated toilet room.
Installation of new partitioned alcove to accommodate relocated service sink in the fish market
along with associated plumbing work.
Installation of wood framed deck over existing recessed lobster tank which is being abandoned.
Please be advised that the alteration work is not substantial in nature given the limited scope of the work.
I estimate that the cost of these alterations will be in the neighborhood of $75,000.00 and the value of the
existing building (structure only, not land) based on the Town Assessors office records is in excess of
$350,000.
All of the above work is shown on the submitted drawings which I trust will be to your satisfaction.
