Patricia C. Moore, Esq. on behalf of JUNE G. WETLAND requests an Amendment to Wetland
Permit #8017 for proposed terracing and re-vegetation including erosion jute stapled to bluff;
coirlogs at base of bluff and coirlogs placed on bluff staked with Oft. hardwood stakes.

The proposed action has been reviewed to Chapter 268, Watertront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standards. Based upon the information provided on the LWRP Consistency Assessment
Form submitted to this department, as well as the records available to me, it is my
recommendation that the proposed action is CONSISTENT with the LWRP.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in
preparing its written determination regarding the consistency of the proposed action.

A. INSTRUCPLONS
1. All. applicants for pertnits* including Town of Southold agencies, shall complete tlils CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This -
assessment is intended to supplement other information used by a Town of Southold agency in
malting a determination of consistency. *Ezcept minor exempt actions including Building Permits '
and ether ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of th{s form.should review the exempt `
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront. Revitalization Frogram. A protwsed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Southold Town,

with associated covered platform down the embankment; construct a new retaining wall; add fill and re-

The proposed action has been reviewed to Chapter 288, Waterfront Consistency Review of the Town of
Southold Town Code and the Local Watertront Revitalization Program (LWRP) Policy Standards.
Based upon the information provided on the LWRP Consistency Assessment Form submitted to this
department, as well as the records available to me that the proposal to reconstruct the existing storm
damaged bulkhead; reconstruct existing stairs to beach; construct a new rotaining wall add fill
and re-vegetate area to stabilize the eroded bank is CONSISTENT with the LWRP
The Board issued a permit for the bulkhead in 1993 and stairs in 2001.

AUTHORIZATION
Pursuant to the provisions of Chapter 275 of the Town Code of the Town of Southold and in ~
accordance with the Resolution of the Board of Trustees adopted at the meeting held on Jet
ary 23.2013, and
in consideration of application fee in the sum of $250.00 paid by Jane G Weilsad and subject to the Terms and
Conditions as stated in the Resolution, the Southold Town Board of Trustees authorizes and permits the ~
following:
Wetland Permit to reconstruct the existing storm damaged bulkhead; j
reconstruct existing stairs to beach; rernnstruct the existing stairs with associated
covered platform down the embankment; construct a new retaining wall; add fill and
re-vegetate area to stabilize the eroded bank; with the coedidon of the inclusion of 16'
returns at each end of lower bulkhead and 6' returns at each end of retaining wall, and
the area between the two structures are to be left natural; and as depicted on the site
plan prepared by Chorno Associates Architects, last dated December S, 2012, and
stamped approved on January 23, 2013; and as also depicted oa the site plan prepared
by Joseph Fischettl, B.E, last dated November 26, 2012, sad stamp approved on
January 23, 2013.
