LYLE 8~ KATHLEEN GIRANDOLA request an Amendment to Wetland Permit #8002 to move
-the proposed upper retaining wall further landward for a total of +/-15 feet from existing lower
bulkhead to be in alignment with neighbor's to the west; raise the height of the proposed
retaining wall 1'-2'; and cap the lower bulkhead with grate-style capping. Located: 3040 Great
Peconic Bay Boulevard, Laurel. SCTM# 128-6-10
The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standards. Based upon the information provided on the LWRP Consistency Assessment
Form submitted to this department, as well as the records available to me, it is my
recommendation that the proposed action is CONSISTENT with the denoted following Policy
Standards and therefore is CONSISTENT with the LWRP.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in
preparing its written determination regarding the consistency of the proposed action.

I. All applicants for permits* including Town of Southold agencies, sh complete this CCAF for
proposed actions that aze subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supglement other information used by a Town of Southold agency in
making a determination of consistency. *Fxcept,minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C, the preparer of this form should review the exempt ~
minor action list, policies and explanations of each policy contained in the Town of Southold Local I'
Waterfront Revitalization Program. A proposed action will be evaluated as to its simificant ~
beneficial and adverse effects~on the coastal area (which includes all of Southold Town).

3. If any question in Section C on this form is answered "yes" or "no", then the proposed acfion will
affect the achievement of the LWRP policy standazds and conditions contained in the consistency
review law. Thus each answer must be explained in detail listing both supaortine and non- i
suaaortine; facts. If an action cannot be certified as consistent with the LWRP policy standards and {

Wetland Permit to remove 68 feet of lower bulkhead and 68 feet of upper i
retaining wall destroyed by Hurricane Sandy; construct new vinyl bulkhead with l2 ft.
return and new vinyl retaining wall with 12 ft. return; top of lower bulkhead to be 18 ~
inches higher than previous bulkhead; top of retaining wall to be 12 inches higher than
previous retaining wall to match neighbor's bulkheads; construct upper and lower
beach stairs and connecting walkway destroyed by Httrrkane Sandy; fill lawn area
with approx. 250 cy. of clean till; restore 11 foot non-turf buffer between bulkhead and
retsuaing waU; replant with native sea grass; and. as depicted on the project plan
prepared by Juditti Crabtree Duffy, rebeivted on December i 1, 2012 and stamped
approved on January 23, 2013.
