tenant in common, and Lisa J. Diana, or her substitutes or successors, as trustee of the
Rosemarie L. Forte Credit Shelter Trust under the probated Last Will and Testament of
Rosemarie L. Forte, deceased, residing at 6 Wendy Lane, East Northport, NY 11731, as a
one-half tenant in common.

party of the second part, does hereby grant and release unto the party of the second part, the heirs or successors and assigns of
the party of the second part forever,

at Cutchogue, Town of Southold, County of Suffolk and State of New York, all as more particularly described on the
attached Schedule A.

1000 Forte, now deceased, by deed dated August 6, 2043 and recorded on August 13, 2003 in the Office of the Suffolk
County Clerk at Liber 12266 Page 690.
Sec. This deed shall serve to confirm the Irrevocable Disclaimer and Renunciation o[ Interest in Estate dated January 13,
102.00 2013 executed by grantor Robert A. Forte, individually, and tiled in the probate file for the Estate of Rosemary L.
Forte in the Surrogate's Court of Suffolk County, NY on February 11, 2013. By virtue of said Disclaimer, grantor,
Robert A. Forte, disclaimed his survivorship interest in the subject premises, allowing the same to pass under the Last
l3lk. Will and Testament of Rosemarie L. Forte, Article THIRD thereof, to the Rosemarie L. Forte Credit Shelter Trust
08.00 established under said Last Will and Testament.

the above-described premises to the center lines thereof; TOGETHER with the appurtenances and all the estate and rights of
the party of the fast part in and to said premises; TO HAVE AND TO HOLD the premises herein granted unto the party of
the second part, the heirs or successors and assigns of the party of the second part forever.
AND the parry of the first part, in compliance with Section 13 of the Lien Law, covenants that the party of the first part will
receive the consideration for this conveyance and will hold the right to receive such consideration as a trust fund to be applied
first for the purpose of paying the cost of the improvement and will apply the same fast to the payment of the cost of the
improvement before using any part of the total of the same for any other purpose.
AND the party of the first part covenants as follows: that said party of the first part is seized of the said premises in fee simple,
and has good right to convey the same; that the party of the second part shall quietly enjoy the said ;that the said premises are
free from enctunbrances, except as aforesaid; that the party of the fast part will execute or procure any further necessary
assurance of the title to said premises; and that said party of the fast part will forever warrant the title to said premises.
The word "party" shall be construed as if it read "parties" whenever the sense of this indenture so requires.
