LWRP CONSISTENCY ASSESSMENT FORM
INSTRUCTIONS
All applicants for permits* including Town of Soathold agencies, shall complete this CCAF for
proposed actions that are subject to the Town ofSouthold Waterf~at Consistency Review Law. This
assessment is intended to supplement othe~ information used by a Town of Southold agency in
making a determination of ennsisteney. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
Before answering the questions in Saction C, the p~parer of this form should review the exempt
minor action list, pelicies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated, as to its
beneficial and adverse effects upon the coastal area (which includes all of Southold Town).

If any question in Section C on this form is answered "yes" or "no", then the proposed action will
affect the achievement of the LWRP policy standards end conditions contained in the consistency
review law. Thus~ each answer must b.e explaint, d in detail~ listin~ both supporting and non
suooortine facts. If an action cannot be certified as consistent with the LWRP policy standards and
conditions, it shall not be undertaken.

October 19, 2005:
RESOLVED, that the Southold Town Board of Trustees APPROVE the Amendment to
Permit # 5047 as follows:
Allow for the removal and replacement (inkind/inplace) of an existing 12.5 x 14' wood
deck behind the bulkhead, as depicted on the survey prepared by Joseph A. Ingegno
last dated September 13, 2005.
This is not a determination from any other agency.
