LWRP CONSISTENCY ASSESSMENT FORM
A. INSTRUCTIONS
All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Southold Town).
If any question in Section C on this form is answered "yes", then the proposed action may affect the
achievement of the LWRP policy standards and conditions contained in the consistency review law.
Thus, the action should be analyzed in more detail and, if necessary, modified prior to making a
determination that it is consistent to the maximum extent practicable with the LWRP policy
standards and conditions. If an action cannot be certified as consistent with the LWRP policy
standards and conditions, it shall not be undertaken.

Nature and extent of action:
Applicant was issued Trustees Permit No. 79~25 on October 17, ~201~ for the demolition/reconstruction
of the single-family dwelling thereon, and wishes to amend stone to include for a Proposed Parking
Area (~2~'+ x ~2~2'+ ) within the northeastern section of the property surrounded by Belgium Block
Curbing (66' L _+), a Proposed Landscape Retaining Wall (1.0' W + x 11.5' L + x ~2.5' H +) & three (5)
Stone Piers (5.0' H) off the southerly edge of the Proposed Parking Area, a Proposed Bluestone
Landing (la5 sq. ft. _+), Bluestone Slab Steps (55 sq. ft. +) and Bluestone Stepping Stones (6" T)
leading from the proposed Steps to the existing Single-Fainily Dwelling.

Proposed Wetland Permit for MURRAY GAYLORD
SCTM#1000-116-4-20.1
Suffolk Environmental Consulting, Inc. on behalf of MURRAY GAYLORD requests a Wetland Permit to
elevate the existing dwelling; construct a front entry; rear deck; screen porch; addition; stoops (2); deck
roof; install new sanitary system; deposit clean fill; demolish the existing garage; and establish a natural
buffer. Located: 765 Beachwood Rd., Mattituck. SCTM#116-4-20.1
The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the Town of
Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy Standards.
Based upon the information provided on the LWRP Consistency Assessment Form submitted to this
departm.ent, as well as the records available to me, it is my recommendation that the action is
CONSISTENT with the LWRP provided the following is addressed:
1. Depth to groundwater is extremely shallow 2.4'. Please have the applicant confirm the Iocat on of
the test well.
2. The 4' contour is proposed "to be removed"? Please verify the intention of the note.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
wdtten determination regarding the consistency of the proposed action.
