Dear Amanda,
Enclosed are the completed "Application for Amendment to a Wetland Permit" and the "LWRP
Consistency Assessment Form" for the repair on my dock located at 35 Haywaters Road,
Cutchogue (Tax Map # 111.-1-4) from the damage incurred during Hurricane Sandy last year. I
am asking to replace the ramp which was completely destroyed and the two spiles that secure the
floating dock in place.
I am also enclosing an updated drawing of the dock. The original drawing in your file (enclosed)
showed a 10' X g' platform. That platform no longer exists. There is only a catwalk that is 3'
wide from the land to the ramp leading to the floating dock.
Also enclosed is a copy of the original "Authorization" for dock permit #1947 and a check for
$50.00.
I trust that all the information you need is enclosed. If you have any questions, please do not
hesitate to contact me at 202 744-1467.
Thank you for your help on the phone.
Sincerely,
