SCTM#1000-111-1-4 ALFONSO MARTINEZ-FONTS, JR. requests an Administrative Amendment to Wetland Permit #1947 for the existing 3'x32'4" fixed catwalk, 3'x12' ramp, and 18'x5' floating dock, and to replace the ramp and the two piles for the float. Located: 35 Haywaters Road, Cutchogue. SCTM# 111-1-4 The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy Standards. Based upon the information provided on the LWRP Consistency Assessment Form submitted to this department, as well as the records available to me, it is my recommendation that the proposed action is EXEMPT from LWRP review pursuant to: § 268-3. Definitions. MINOR ACTIONS item "A" which states: A. Maintenance or repair involving no substantial changes in an existing structure or facility; Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its written determination regarding the consistency of the proposed action. Box 1179 Southold, NY 11971 Att~: Amanda Nunemaker Dear Amanda, Enclosed are the completed "Application for Amendment to a Wetland Permit" and the "LWRP Consistency Assessment Form" for the repair on my dock located at 35 Haywaters Road, Cutchogue (Tax Map # 111.-1-4) from the damage incurred during Hurricane Sandy last year. I am asking to replace the ramp which was completely destroyed and the two spiles that secure the floating dock in place. I am also enclosing an updated drawing of the dock. The original drawing in your file (enclosed) showed a 10' X g' platform. That platform no longer exists. There is only a catwalk that is 3' wide from the land to the ramp leading to the floating dock. Also enclosed is a copy of the original "Authorization" for dock permit #1947 and a check for $50.00.
