This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: I. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
I. Accurate survey of property showing ali property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. Ifa Certificate of Occupancy is

RE: MURRAIN WITH NORTH FORK BANK
Dear Mrs. Murrain:
My office was contacted by John Boufas, Southold Town Building Inspector, on May 9. He
advised that an "AS BUILT" certificate of occupancy could be issued for the dwelling with an
unihabitable basement, upon completion of the following items:
· Installation of five (5) battery-operated smoke detectors (1 in each bedroom, 1
outside of bedrooms and 1 in basemen0;
· Remove the gas heater;
· Obtain an electrical underwriter's certificate;
· Chimney flew must be "smoke-fight" and screwed together at the joints;
· If flew pipe is less than 18" below the floor 5/8" protection sheetrock must be
placed around the floor; and
· The basement must be cleaned up of any and all debris, i.e. ceiling tiles.
