YELJKO J. I~{STULOVIC, 609 WEST ti4TH STREET, NEW YORK, NEW YORK 10025 for a Department of the Army approval of existing pier, float and piles in MATTITUCK CREEK, AT SHORE ACRES, TOWN OF SOUTHOLD, SUFFOLK COUNTY, NEW YORK. A copy of the plans of the work is shown on the reverse of this sheet. The applicant has constructed a pier extending 20 feet offshore of an existing bulkhead and having a dock elevation of 2 foet abo~ the plane of mean high water. The applicant has also installed a float held in place by piles having a top elevation of 1 foot above the plane of mean high water and connected to the existing pier by means of a ramp. The entire structure extends 65 feet offshore of the existing balkhead. The decision as to whether approval will be granted will be based on an evaluation of the impact of the work on the public interest. Factors affecting the public interest include, but are not limited to, navigation, fish and wildlife, water quality, economics, conservation, aesthetics~ recreation, water supply~ flood damage prevention~ ecosystems, and, in general, the needs and welfare of the people° Any criticisms or protests regarding the work should be PREPARED IN WRITINC AND MA~ED TO KEACR THIS OFFICE prior to 8 January 1973 otherwise it will be presumed that there are no objections. In addition, this application is limited to only previously Trustee permitted structures from 1991 to date and that have or are accompanied by detailed plans/drawings. All other structures, permitted or not, prior to 1991 do not qualify for this expedited permit process and will be subject to the normal appropriate permit application process. This is not a determination from any other department or agency. The following items are required: 1. Photographs of the area prior to the storm. 2. Photographs of the area showing the storm damage. 3. A copy of the latest survey of your property, if available. 4. A completed emergency application form. Upon issuance of Emergency Permit the following Best Management Practices must be followed: A 15 foot Non-Turf Buffer must be installed and subsequently maintained along the landward edge of the bulkhead after completion of repairs/reconstruction. Requests for exceptions to this requirement must be made in advance to the Trustees. No fill may be placed on any existing vegetation, as per Chapter 275 of the Town Code. All construction/work must otherwise conform to the relevant provisions of the Town Code. Untreated lumber must be used on any stair treads, handrails and decking.
