Re: Amendment to Permit
District 1000 ........ ,,-,, P_,',ock ~,~, Lct .... '1
Anne and Samuel Polk - Owners
To Whom It May Concern:
The enclosed drawings A-l, A-3, A-4, A-5 and A-6 reflect changes to our project as
follows and replace the drawings of the same numbers on file. Changes are as follows:
1. The owners want one story rather than two stories on the east wing.
2. Rebuild the existing portion of the house and replace the existing deteriorated
foundation with a poured foundation and connect to the existing in place septic
just as before by filing a repair to the septic.
Our contractor, B D Remodeling, says it would be easier and better to rebuild the
existing portion of the house rather then try to repair it. The block foundation is
deteriorated allowing the intrusion of termites and rodents. The structure is constructed
of undersized joists, rafters and studs (2x4) materials. Upgrading to code would be very
difficult.
We want to stick with our design, but accomplish it by rebuilding the existing portion
over a new poured foundation.
Lot coverage, setbacks and height remain in compliance. The footprint is similar. The
number of bedrooms (four) remains unchanged but there is one less powder room. One
of the four bathrooms is now located in the basement which is 50 percent above grade
where a sleeping room is shown.
We hope that you can approve our amendment so that we can get building shortly.
Thank you,
Susan E~. ¥0ung'j'j'~.A.

Air Leakage:
F~ Joints, penetrations, and all other such openings in the building envelope that are sources of air leakage are sealed.
Recessed lights are 1) Type lC rated, or 2) installed inside an appropriate air-tight assembly with a 0.5" clearance from
combustible materials. If nor~tC rated, fixtures are installed with a 3" clearance from insulation.
Vapor Retarder:
I~1 Installed on the warm-in-winter side of all non-vented framed ceilings, walls, and floors.
Materials Identification:
Materials and squipment are lastaEed la accordance with the manufacturer's instetlati(m instructions.
Materials and equipment are identified so that compliance can be determined.
Manufacturer manuals for all installed heating and cooling equipment and se~ice water heating equipment have been provided.
Insulation R-values and gtazing U-factors are cleady marked on the building plans or speetfications.
Insulation is installed according to manufacturer's instructions, in substantial contact with the surface being insulated, and in a
manner that achieves the rated R-value without compressing the insulation.
Duct Insulation:
Supply ducts la uncenditior~ed attics or outside the building are insulated to R-8.
Return ducts la unconditioned atlics o~ outside the buttding are insulated to R-4.
Supply ducts in unconditioned spaces are insulated to R~.
[] Return ducts in unconditioned spaces (except basements) are insulated to R-2. Insulation is not required on retum ducts in
basements.
Duct Conctructlon:
I-t Ail joints, seams, and connections are securely fastened with welds, gaskets, mastics (adhesives), mastic-plus-embedded-fabric,
or tapes. Tapes and mastics are rated UL 181A or UL 181B.
Exceptions:
Continuously welded and locking-type longitudinal joints and seams on ducts opereting at less than 2 in. w.g. (500 Pa).
[] The HVAC system provides a means for balancing air and water systems.
Temperature Controls:
I~ Each dwelling unit has at least one thermostat capable of automatically adjusting the space temperature set point of the largest
Electric Systems:
r]l Separate electric meters exist for each dwelling unit.
Fireplaces:
[] Fireplaces are installed with tight rRting non-combustible fireplace doors.
Fireplaces have a s~rce ~f c~mbusti~ air~ as required by the Firep~ace c~nstruc~n pr~visi~ns ~f the Bu#ding ~de ~f New Y~rk
State, the Residential Code of New York State or the New York City Building Code, as applicable,
Service Water Heating:
Water heatem with ver~cal pipe dsers have a heat trap on both the inlet and ouflat unless the water heater has an integral heat
trap or is pert of a cimulating system,
[] Circulating hot water pipes are insulated to the levels in Table 1.
Circulating Hot Water Systems:
[] Circulating hot water pipes are insulated to the levels in Table 1.
Swimming Pools:
[] All heated swimming pools have an on/off heater switch and a cover unless over 20% of the heating energy is from no~-deplatable
sources. Pool pumps have a time clock,
Heating and Cooling Piping Insulation:
[] HVAC piping conveying fluids above 105 degrees F or chilled fluids below 55 degrees F are insulated to the levels in Table 2.
