Notwithstanding any contrary term in the Agreement, the parties agree to add the following to the Service Agreements. Protection of Customer CPNI. a) Verizon will protect the confidentiality of Customer CPNI in accordance appliceble laws, rules and regulations. Verizon may access, use, and disclose Customer CPNI as permitted or required by applicable laws, rules, and regula~ns or this Agreement. b) Provided that Customer is served by at least one dedicated Verizon representative under the service Agreements (that can be reached by Customer by means other than calling through a call center) and as permitted or required by applicable law, Verizon may provide Customer CPNI (including, without restriction, call detail) to representatives authorized by Customer ("Authorized Customer Representatives" as defined below) in accordance with the following: c) Verizon may provide Customer CPNI to Authorized Customer Representatives via any means authorized by Verizon that is not prohibited by applicable laws, rules, or regulations, incJuding, without restriction: to the Customer's email address(es) of record (if any) or other email addresses furnished by Authorized Customer Representatives, to the Customer's telephone number(s) of record or other telephone numbers provided by Authorized Customer Representatives, to the Customer's postal (US Mail) address(es) of record or to other postal addresses furnished by Authorized Customer Representatives, or via Verizon's on-line customer portal or other on-line communication mechanism. d) Authorized Customer Representatives incJude Customer employees, Customer agents, or Customer contractors, other than Verizon, who have existing relationships on behalf of Customer with Verizon customer service, account, or other Verizon representatives and all other parsons authorized in written notice(s) (including email) from Customer to Verizon. Authorized Customer Representatives shall remain such until Customer notifies Verizon in writing that they are no longer Authorized Customer Representatives as described below. Customer's notices of authorization or deauthorization must be sent to your service or account manager, and must contain the following information: the name, title, postal address, email address, and telephone number of the person authorized or deauthodzed that the person is being authorized, or is no longer authorized, (as applicable) to access CPNI the full corporate name of the Customer whose CPNI (and whose affiliates' CPNI) the person can access (or can no longer access as applicable), Dudng the Service Agreements, Customer will at all times have designated, below, in an attachment containing the same data elements listed below, or in a separate writing sent to the service manager or account manager, representatives ("CPNI Authorizers") with the power to authorize Customer representatives to access CPNI under this Agreement, and who are themselves authorized to access CPNI of Customer and its affiliates. Additions or removals of CPNI Authorizers will be effoctive within a reasonable period after Vedzon has received a signed wrrdng of the change, including the affected person(s)' name, title, postal address, email address, and telephone number. 2. The defined terms set forth in the preamble and recital clauses of this Agreement are deemed contractual. 3. If Customer later wishes to select another telephone company, it may be required to pay a reconnection charge to that company. Customer appoints Verizon to: (a) Act as Customer's agent in order to effectuate the preferred carrier change, including the collection of account information, such as the retrieval of the Customer Service Record (CSR) from Customer's current Local Services Provider if Local Services and/or VolP Inbound Local Origination is selected, and to carry out the changes(s) authorized herein on Customer's behalf. (b) Order 1+ access with the local telephone company for lines associated with the BTNs listed above and designated on this form or its attachments. (c) Handle all negotiations for service requests with the local telephone company, interconnecting company and any other equipment supplier, including access service requests (ASRs), requests involving equipment records, billing inquiries, and the issuance cf orders related to Customer's BTNS listed above and associated telephone numbers. This Letter of Agency is in addition to any other agency agreements currently in effect and does not preclude Customer from acting on its behalf when deemed necessary. This agency designation supersedes previous agency agreements with respect to the services identified herein. This Letter of Agency is effective from the date written below until termination or revoked by Customer in writing to Verizon. Customer understands and accepts the terms and conditions of this Letter of Agency. Communications services provided as a result hereof will be governed by Verizon state and federal tariffs and Verizon's Service Publication and Price Guide, as applicable. Customer's representative signing this Letter of Agency is duly authorized to act on Customer's behalf and bind Customer to this agency agreement. ELECTRONIC SIGNATURE. Customer may sign this Letter of Agency by emailing it to Vedzon, in accordance with Verizon's instructions and the Electronic Signatures in Global and National Commerce Act, as it may be amended from time-to-time (the "E-Sign Act"). Customer agrees that any Services provided hereunder may not be used primarily for personal, family, or household purposes.
