The Office of the State Comptroller Division of Local Government and School Accountability is registered
with the NYS Education Department as a sponsor of continuing education courses.
Our sponsor identification number is 000593. Attendees subject to CPE requirements, who wish to receive
CPE credit, should use this form to obtain credit.
During the conference- Use this form to keep track of the sessions you attend. Please sign the attendance
register available at each session. If you do not sign the attendance registers at the time the sessions
are held, no credit will be given for that session.
At the conclusion of the conference- Please complete this form and submit it to an OSC representative
(registration desk, consulting booth), or mail it to OSC. We will mail you the certificate for sessions
attended based on our attendance registers. You should then keep the certificate as a record of your
attendance.
