15. Suffolk Environmental Consulting, Inc. on behalf of ROBERT SWING requests a
Wetland Permit to construct an addition to the landward side of the existing dwelling,
containing 627 sq.ft+/-, removal of the existing septic system and replace with an
updated sanitary system within the front yard, installation of a retaining wall (surrounding
the sanitary system) measuring 118'+/- in length × 1.3' in height, installation of a French
drain along the southern side yard property boundary and establishment of a 5' wide
non-disturbance buffer along the landward side of the bulkhead. Located: 4295

Wetland Permit to construct a 1,665 sq.ft, second-story addition to the existing dwelling;
a 127 sq.ft, second-story and 174 sq.ft, one-story addition off the northern face of the
existing dwelling; a 206 sq.ft, second-story addition addition, 190 sq.ft, porch and 162
sq.ft, steps off the northeastern face of the existing dwelling; a proposed 220 sq.ft.
driveway expansion off the southwestern existing driveway; and a new sanitary system.

of existing northerly return to be removed; replace existing wood fence; remove existing
return landward of existing dock to be temporarily detached during construction; re-
sheath approx. 655 linear ft. of existing bulkhead below the lower wale on its seaward
side with plastic sheathing; and backfill with approx. 200 cy. clean sand to be trucked in

20. En-Consultants, Inc. on behalf of KIMBERLY MUELLER requests a Wetland Permit to
construct a 4'X 30' fiberglass grid catwalk (2' above grade of marsh) with "L"-shaped 4'X
8' stairs at the seaward end. Located: 1445 Bungalow Lane, Mattituck. SCTM#123-3-19
21. Costello Marine Contracting Corp. on behalf of MICHAEL SLADE requests a Wetland
Permit to remove 163' of existing wooden boardwalk to allow for the reconstruction of
the existing bulkhead. Reconstruct 172' of existing bulkhead by re-sheathing landward
side of bulkhead with "Everlast" 2.1 vinyl sheathing. Install new I"X 6' tie rod ends
welded into existing backing system tie rods. Reinstall wooden boardwalk in-place after

bulkhead reconstruction is completed. Construct an 8' extension to existing finger pier.
Install two new 10" dia. X 30' long support pilings at offshore end. Maintenance dredge
an area 50' seaward from the existing bulkhead to a depth of -4.0' below MLW on the
east end and progressing to -7.0' below MLW on the west end. Approx. 350 cy. of
dredged spoil to be trucked off site to an approved upland disposal site. Located: 1435
