INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
£ Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or bas not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
piupe~ty have been enacted in the interim,, thc Building inspector may attth~hze in writing, the extension of the permit fYr an
addition six months. Thereafter, a new permit shall be required.

Dear Ms. Foss:
The Suffolk County Water Authority has received your application for water service for the
above referenced parcel. The parcel is desigaated in the SCWA's Five Year Water Supply Plan
for the North Fork as eligible to receive public water service. Accordingly, this Letter of Water
Availability is issued. There is an existing main located on Liberty Lane which can serve this
parcel.
This Letter of Water Availability is not to be considered an action by SCWA as defined by the
New York Environmental Quality Review Act or a determination that the parcel is approved for
building. You are responsible for obtaining any other permits or approvals that are required for
the proposed project. SCWA shall not be required to provide water for the parcel until all of the
other approvals have been obtained.
SCWA will provide water service to the parcel upon payment of the applicable fe~s. The fees
include a tapping fee, a key money charge and if applicable, a water main surcharge. Tapping
fees start at $950.00 for a 1" service. Key money fees start at $3,000.00 for a 1" service.
Surcharge amounts vary depending on the location of the proposed service. The tapping fees and
the surcharge may be financed for existing residential customers.
