This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial build'rog, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consem to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Upon receipt of an application for a Pre-Existing Certificate
of Occupancy, I made an inspection of the building at this location.
I picked up keys from Floyd F. King Jr.~ Real Estate Office, Main
Road~ Orient~ New York and began this inspection at approximately
10:10 A.M. ~he dwelling is a one story~ wood framed structure
constructed on cement block foundation. There are two bedrooms~
a full bathroom~ kitchen and living room with a fireplace on
exterior west wall. Heat is furnished to building from a gas fired
warm air furnace with heat outlet located in living room floor
approximately centered in building. An enclosed porch runs entire
width of front of building and a partially enclosed porch protects
rear entry to kitchen.
The following violations of the Housing Code~ Chapter 52~ Code
of the Town of Southold: New York were found:
Front Porch : flooring~ to left of entry door along front
of porch~ is rotted away and weakened in some areas.
Article III~ Section 52-30A.
Gas Cookin~ range: no shut off valve at unit. Article V,
Section 52-52 B.2.
Cellar Entry: exterior~ wood rotted and deteriorating.
Article V~ Section ~2-30 A.
Stairwell: for exterior cellar stairway-cement blocks have
cracked and motor joints have opened~ stairwell walls are
deformed. Article III~ Section 52-30 B.

installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

Here is an application for a certificate of occpuancy
on a certain dwelling owned by Leonard Gordon and located
on the northerly side of Main Road in Orient. Also
enclosed are a survey print, check for $5.00 and
affidavit.
The key is at the office of Floyd F. King, Jr., realty,
22420 Main Road, Orient, and if you will be kind enough
to telephone Wilma Hoffmeister of that office at business
number 323-2570 or residence 765-5532, she will make the
key available to you for an inspection. The house is now
vacant.

located on the northerly side of Main Road, Orient,
New York which is located between land of Gilbert on the west
and land of Ethier on the east. He purchased the premises from
GLADYS WILKINS on August 26th, 1971 and has owned the same
premises ever since that time.
3. To the best of my knowledge, the building is more
than fifty years old and has always been used as a one-family
dwelling. The condition of the building is suitable and is
now in habitable condition for one-family residential purposes
4. This affidavit is submitted in connection with my
application for a "pre-existing" certificate of occupancy.
