I am the owner of the property known as 1080 Deep Hole Drive, Mattituck, NY 11952, SCTM#
1000-11500-1200-15000. I hereby give my permission for the Trustees to enter the property for
the purposes of viewing the existing dock structure. Our dock was grandfathered on permit #
4778. Please inspect grandfathered dock and provide a Certificate of Compliance. The house is
being sold. The pumhaser, Frank Marsilio, seeks pm-submission inspection and the Trustees'
verbal opinion regarding whether or not it will be acceptable to make an addition and/or changes
to the existing structure. Mr. Marsilio and I will split the $50 fee in regard to this visit by the
Trustees and will submit such.

in Southold, can be reached at 765-8161 or 765-3416, in regard to any arrangements necessary to
coordinate your visit to the property at your next scheduled inspection date of January 13, 2010.

a 6'x20' floating dock and ramp to the existing dock or what modifications would need to
be made in order to accommodate a 24' to 30' boat.
