This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of Property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6, Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:

f. Every building pemfit shall expire if the work authorized has not commenced wiflfin 12 montbs a~er the date
issuance or bas not been completed within 18 months from such date. If no zoning amendmems or otber regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

SPEClFICATIONS
GENERAL CONDITIONS
1) The contractor is responsible for checking aft dimensions and conditions on
the job. Where conditions prevent obtaining dimensions they should be
checked by engineer.
2) Construction contractor and his subcontractors agree that in accordance
with generafty accepted construction practices, construction contractor and his
subcontractors will be required to assume sole and complete responslbiftty for
job site conditions during the course of construction of the project including
safety fo oll persons and property and that this requirement shall be made to
apply continuously and not be limited fo working hours. Contractors agree to
defend, indemnify and hold design professionals hormtsss from any and oft
ftabiftty, real or afteged, in connection with the performance of work on this
project.
$) All work performed by the contractors shall conform to the requirements of
local, municipal, state and federal laws os well as any other governing
requirements, whether or not specified on drawings or in specifications.
Contractor to be responsible for required inspection(s) of his work.
The contractors shall notify the architect/engineer immediately if he cannot
with the work called for on the drawings.
5) The contractors shall notify fha architsct/engineer of any discrepancies
between fha drawings, notss, specifications and field conditions before
commencing any work and request clarification.
6) Contractor to verify new work with survey.
Contractor shall be responsible for culling, fitting or patching to complete
work or make the ports flf together properly.
8) The contractor shall be fully responsible for the removal or replacement of
damaged or defective material or poor workmanship. They shaft replace or
repair as directed by the architect/engineer, all such damaged or defective
materials, which shall appear within a period of two years from the dote of
substantial completion of the job.
The contractor(s) shall remove oft rubbish, waste matsriots and surplus
materials that ore a by-product of their work.
10) Contractors shall provide aft necessary protection for his workers.
11) The construction drawings and specifications are supplied fo illustrate the
design and the general type of construction desired and ore intended to imply
the finest quality of construction, material and workmanship throughout.
12) The contractor(s) shall maintain a current and complete set of
construction drawings on the site during oft phases of construction for use by
all trades.
13) No changes or modifications to the work shaft be made without approval
of the owner and/or architect/engineer. Failure to obtain this approval shall
cause the contractor or sub-contractor to assume full responsibility for
subsequent modification of the work required by owner or any regulatory
authority.
14) The architect/engineer assumes no responsibility tar construction means,
methods, techniques, sequences, or procedures or for safety precautions and
progress in conjunction with the work. There are no warranties, nor any
merchanfobftity of fitness, nor o specific use expressed or implied in the use
of these plans.

I) Wood frame dwellings to be designed in accordance with the Building and
Residential Code of New York State. Wind speed to be used is 120 mph.
Exposure ft category to be used for urban, suburban or wooded and Exposure
to be used for open and shoreline oreo& Earthquake Use Group I, Sits
D, Seismic Design Category B.
2) These plans ore designed to meet or exceed the requirements of the
Building and Residential Code of New York State. When building in other
jurisdictions, variations or adjustments may be required. Verify any such
requirements w th oco code or enforcement officers.
The architect/engineer's certification applies only to this plan's conformance
fha New York State Building & Residential Code.
4) Glozed openings must be protected with gloss, certified fo meet fha large
missile test Ompacf-restsfonf) or with structural shutters with aHachment
hardware provided.
5) Electrical work shall conform to notional electrical code (latest edition) and
local codes. A minimum of one (1) smoke detector shaft be
in bedroom areas and shall be hard wired and bo#ery backup.

requiring a building permit requiring o building permit requires that oil
sleeping rooms in the house be upgraded with hard-wired and interconnected
smoke alarms.
Contractor is responsible for aft flashings.
Excavations deeper than 5' must be constructed according to 05HA
regulations. Soft or sandy soils must hove of least o 1:1 slope. Hard or
compact soils must hove at least 1:2 slope.
15) Grade property 5~ in first 10' from the building. Elsewhere fha site to
be sloped away at 29[. Foundation wall to extend o minimum 8" above grade.
14) Under slab areas - after the service lines have been installed, aft plastic
or copper lines should be properly bedded (cleon fill, no rocks) and tested for
leaks prior to backfilling. All copper lines should be checked for dents or
abrasions and wrapped in spftf foam insulation.
15) All trades that are installing piping under the slab must install color coded
caution tope 12" above their work. Use red for electric, blue for wafer, green
for sewer, yellow for gas and orange for phone/cable. Mark location of new
work with color coded spray po nt offer trenches have been bockfgled.
16) Copper joints will not be allowed under the slab and must be placed in a
waft manifold.
ENERGY NOTES
l) This construction conforms to Residential Code of NYS, Chapter 11 and the
Energy Conservation Construction Code of NYS.
2) Model Energy Code compliance method and inspection checklist, enclosed
using MECcheck software type Version 3.5 Release 1, REScheck Software.
3) A minimum of 1~ air space shall be provided between fha insulation and
roof sheathing.
CONCRETE SPECIFICATIONS
1) Interior and exterior footings, slobs, and wafts shall be 3000 PSI concrete
unless otherwise noted.
Footings below columns fo be 24"x24~x12" unless otherwise noted.
3) Contractor fo verify soil bearing capacity to be not less than 4000 PSF.
No footings or slobs ore to be cost on uncontrolled fill, loose soil, organic
material, frozen ground, mud, cloy or other unsuitable materials. If any of
these situations are present it should be brought to the aNention of the
engineer.
4) Contractor to verify existing and seasonal ground-water level. If level is
within 2 feet of the boNom of fha basement footing the orohitect/engineer
should be contacted.
5) Concrete shaft be oir-enirained with an air content not less than 59[ or
more than 7~.
6) Concrete slump shall not be more than 5'. Engineer has approval to reject
concrete if slump is larger than specified.
7) If concrets is delivered in on outdoor temperature lower than 40 degrees F
it should be heated fo insure that it will arrive af the job site having a
temperature of not less than 60 degrees F or greater than 85 degrees F.
8) If it looks as though the temperature of the ambient air will fall below
freezing for $ consecutive days, cold weather concrete specifications will need
to be followed. Contact Engineer if this is going to occur.
9) The addition of any admixtures fo concrete shall be approved by the
Engineer.
10) Two concrete strength test cylinders, 6" in diameter by 12' shall be
token from each truck delivery unless otherwise approved by the Engineer.
Compressive strength tests to be taken for each day's pour exceeding 5
cubic yards. Two specimens fo be tested @ 28 days.
1 I) All tabors to be grade 60 KSI stsel and shaft be free of rust, scots and
diri.
12) Aft tabors shaft be securely tied in place and adequately supported. All
tabor splices to be minimum 40 bar diameter. Rebar shall be continuous
through corners with splices located not closer than 3' from corners.
13) Aft footings are fo have their rebar placed no closer than 3' to the
supporting soil.
14) No weight shaft be placed upon footings or walls until 7 days offer
placement os long as the concrete strength has reached 2500 psi or by
engineer's approval.
15) Contractor to notify Engineer for inspection of all tabor work prior to
placement of concrete.
16) Concrete pours over 9' shaft be thoroughly consolidated by suitable means
during placement using o mechanical vibrator,
17) Back filling against foundation shaft not fake place until first floor ion
anchored fo foundation. Bockflft material shaft be cleon course sond.
Equipment used for backfilling shall not come closer fo fha wall than fh~e
height of the backfill.
18) All footings to rest on virgin undisturbed soil. If soil is disturbed,
mechanical compaction to 95~ relative compaction in soil lifts not exceeding
6".
19) Concrete wall design assumes backfill is well drained.
20) Anchor bolts to be 5/8" x 12' "J" shaped, hot dipped galvanized or
stainless steel located 4'on center unless otherwise noted and o maximum of
12" from all corners. Provide not less than two (2) anchor bolts on any one
sill. Anchor bolts to have nut and washer tightened fo sill. Washers shay be o
minimum of 3/16" x $' x 3' square plate washers unless otherwise shown.
Bolts to be Iocotsd down fha center of the sill plate. Deviation of more than
1' will not be acceptable. A hot dipped galvanized or stainless steel 5/8' x
tO" threaded rod con be substituted for anchor bolts by being epoxied into
place using Simpson Acrylic Tie.
21) Contractor to verify with owner if damp-proofing or waterproofing is fo
be provided on oll foundation walls and footing base that comes in contact
with soil.
22) Provide (2) #5 tabors continuous reinforcing at top of oft foundation walls.
Set tabors into concrete with on embedmenf of 3" fram the top and inside
edge and the other bar 3" below fha upper unless otherwise shown.
2,t) Provide (2) //5 robots continuous reinforcing in wall footings placed
from soil and 4' from outside footing edge unless otherwise shown.

30) Welded wire mesh should be positioned in fha top 2" of the slob. Flat

31) Under the slob shall be placed 2' of sand over a vapor retarder -
Fortiflber "Moistop Ultra A' (www. fortiflber, com). Contractor verifies that he has
reviewed the installation guide for this product.
52) Sub-grade preparation under slob - The existing sub-grade should be
cleared of fop soil and organic moiler and stockpiled separately. Pronounced
soft and hard spots should be excavated and refilled and compacted with o
soil similar fo fha surrounding soil.
3S) Granular fill, such as gravel or a mixture of sand and grovel shall be
used as fill under the vapor barrier. If soil is disturbed, mechanical
compaction to 95~ relative compaction in soil lifts not exceeding 8".
34) Curing of concrete slobs open to the elements is to be done by keeping
fha slab wet and at tsmperafures between 40 and 70 degrees F or using a
curing compound.
35) No vehicles shall be placed on the slab until the 28 day strength is
achieved.
FRAMING NOTES
I) All framing lumber to be Douglas Fir //2 or belier in an S-Dry condition.
2) Aft sill plates that sit on concrete to be double 2x6 pressure treated.
Fastening of rim board to sill to be 10d toe nails 6" on center.
3) Termite shield shall be either copper or COP -R-TEX and extend 1"
minimum on each side of the concrete wall. Sill sealer to be used with this
product.
4) Two rows of blocking to be installed every 4' along concrete walls where
the first floor joists are running paroftel to the foundation wall The plywood
floor is to be nailed info this blocking.
5) Fastening of exterior plywood to rim joists (box beam) and double top
plates to be 2 rows of 8d nails 3" on center. Fastening of plywood to double
sill plate to be 3' on center staggered into each 2x member.
6) Fastening of top and bottom plate on interior wafts to be (2) 16d nails
every 12". Top plate fo be connected to ceiling joists or with blocking spaced
not more than 12" on center with (2) 16d nails every 12".
7) All deck ledgers fo be Pressure treated 2x matsrioL Ledgers fo be
attached with ~' lag bolts 12" O.C. top and boflom and located not closer
than 2' from top edge.
8) All headers fo be (2) 2x12 or (2) I-3/4 x 11-7/8 microloms unless
noted otherwise.
9) Double floor joists shall be located under all poraftel partitions unless noted
otherwise. Spacing between floor joists should allow for plumbing in plumbing
walls.
10) All floor joists must be restrained at the ends to prevent rotation by
either a rim joist, end blocking or joist hangers supporting the depth of fha
joist.
I1) Floor plywood to be glued and nailed to floor joists 4" on edges and 6"
in field with 8d common nofts.
12) All walls over 8' high fo be blocked 0 mid-span.
I$) All post widths to be as wide or wider than multiple member width that
it is supporting.
14) 2x6 strong backs (2) on oft ceiling joists 8' O.C., unless the ceiling is an
engineered floor I-joist.
15) Splices of top plates shall not be closer than 6'. Studs to fall under oft
splices, top and boffom. Studs to line up verticofty with roof rafters wherever
possible.
16) All gable end walls to be balloon framed unless otherwise shown.
17) Exterior wall ply to cover roof top plate completely and be nailed with 8d
common nails 3" on center, staggered into both fha top and bottom plots.
18) All roof and wall plywood to be nailed with 8d nails 4' O.C. on edges
and 6" in the field unless otherwise notsd. HoB heads shall not penetrate
surface of plywood. Nail Guns need to be set fo the proper pressure.
19) Exterior plywood shall be continuous over oft plate to plate and plate to
rim board interfaces and may butt together of mid-depth on rim board.
Exterior plywood fo run continuous from sill plots over rim joist to studs. 'L"
shaped plywood fo be placed around upper window and door corners.
20) Three rows of blocking to be installed every 4' along gable rake walls.
The plywood roof is fo be nailed info this blocking.
21) All roof rafters to be connected fo fop plate with Simpson H2.5 or H2. SA
hurricane ties unless otherwise shown. Hip rafters to be tied to fop plate with
Simpson HCP4 (double hip) or HCP1.81 (single hip). Note that portiofty
enclosed structures shaft use a Simpson HIO hurTicane fie.
22) Interior shear walls to be ~" plywood both sides, nailed with 8d nails
O.C. on edges and 6" in field with oft edges blocked and nailed unless
otherwise noted.
23) All exterior columns to have condnuous galvanized metal straps from
girder below to girder above. Straps to be Simpson CS 16 and connected
with (11) 1Od common nails each end.
24) Columns resting on concrete fo be connected at the base concrete with a
Simpson Post Bose fastener and the top with a Simpson type Post Cop with
