Findings: The project meets all the requirements for issuance of an
Administrative Permit set forth in Chapter 275 of the Southold Town Code. The
issuance of an Administrative Permit allows for the operations as indicated on the
application received on April 27, 2012 and as depicted on the site plan stamped
approved on May 16, 2012.
Inspections: Final inspection.
If the proposed activities do not meet the requirements for issuance of an
Administrative Permit set forth in Chapter 275 of the Southold Town Code, a
Wetland Permit will be required.
This is not a determination from any other agency.

Dear Mr. Hobbs:
The Department of Environmental Conservation (DEC) has reviewed your request to modify the
above referenced permit in accordance with the Uniform Procedures Regulations (6NYCRR Part
621 ). It has been determined that the proposed modifications will not substantially change the scope
of the permitted actions or the existing permit conditions. Therefore, the permit is hereby modified to
authorize:
Terrace the property, extend the patio and install an outdoor shower with a drywell. All
work must be done according to the plan prepared by Hobbs dated 6/15110 and stamped
NYSDEC approved on 7/7/10.
Conditions 3 and 4 have been modified and conditions 11 and 12 have been added as follows:
3-Prior to commencement of any construction activities, a continuou.s row of straw
bales shall b® staked end to end downslope of any areas of ground dfsturbance.
4-Straw bales shall be entrenched two to four inches into the ground.
11-The bales shall b.e maintained, repaired and replaced as often as necessary to

LWRP CONSISTENCY ASSESSMENT FORM
A. INSTRUCTIONS
All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfzont Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list, policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated as to its significant
beneficial and adverse effects upon the coastal area (which includes all of Southold Town).
If any question in Section C on this form is answered "yes" or "no", then the proposed action will
affect the achievement of the LWRP policy standards and conditions contained in the consistency
review law. Thns~ each answer must be explained in detafl~ listing both supporting and non-
