A certified copy of this resolution is enclosed with provisions for the event listed that must followed. An insurance policy naming the Town of Southold as additionally insured has not been filed with this office. Sincerely, Lynda M Rudder Deputy Town Clerk enc Southold Town Board - Letter Board Meeting of May 22, 2012 RESOLUTION 2012-443 ADOPTED Item # 5.15 DOC ID: 7879 THIS IS TO CERTIFY THAT THE FOLLOWING RESOLUTION NO. 2012-443 WAS ADOPTED AT THE REGULAR MEETING OF THE SOUTHOLD TOWN BOARD ON MAY 22, 2012: RESOLVED that the Town Board of the Town of Southold hereby grants permission to the Southold School District to use certain roads for a 5K race in Southold~ on November 5~ 2012 beginning at 8:30 AM, route for the 5K: beginning at the Elementary School parking lot onto Oaklawn Avenue to Route 25, east to Youngs Avenue, to Calves Neck Road, to Harper Road, Hill Road, Wells Road, back to Oaklawn Avenue, to Pine Neck Road to Main Bayview Road to Main Road, Jockey Creek Drive back onto the Southold School property, providing 1. They file with the Town Clerk a One Million Dollar Certificate of Insurance naming the Town of Southold as an additional insured; 2. Coordinate traffic control upon notification of the adoption of this resolution with Captain Kruszeski 3. No objects of any kind shall be thrown to event spectators 4. No permanent markings be placed on town, county or state roads or property for the event; 5. Any road markings or signs for the event be removed within twenty-four (24) hours of the completion of the event. Support is ~br this year only, as the Southold Town Board continues to evaluate the use of town roads. All fees have been waived for this event. Elizabeth A. I 201 outhold Town Clerk To The Southold Town Board, My name is Peter Salerno and I am a physical education teacher at the $outhold Elementary School. I have held this position for 13 years and have tried to do my part in promoting health and wellness to the children of the Town of Southold. I am looking for permission to extend this message of wellness into our community by hosting the 3rd Annual Southold Athletic Association 5k race to benefit the health and wellness of the students of the Southold School District. I would like to schedule the event for Saturday, November 10th, 2012. The 5k would begin at 8:30a.m. and be completed by 9:30a.m. I have included a copy of the intended route to be used for the 5k and understand the Southold Town Police will have final approval of the course. All necessary insurance will be obtained upon approval. I appreciate your past support and cooperation. Thank you for your time and consideration.
