APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. Ifa Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Enclosed Please find an application for a Pre-existing Certificate of Occupancy for a Guest House with
attached storage garage located at the above address. This property has been in the Mudd family since
May 14th 1960 as indicated on the enclosed deed Liber 4811 P 366. The Mudd family purchased this
property with the improvements as indicated on the enclosed survey by Peconic Surveyors dated
February 29, 2012.
An earlier survey previously submitted for the purposes of obtaining a swimming pool permit # 11207
dated June 25, 1981 only indicates this structure as a garage. The indication on the survey of the structure
of the garage is merely a note on the survey and does not indicate the entire structures use and occupancy
at the time the survey was completed.
1 have attached for your use documentation from the adjacent neighbors that this structure has had dual use
since prior to the inception of zoning. We respectfully request an inspection of the premises and an issuance
of a Pre-exiting Certificate of Occupancy with a use as a guest house with cooking, bathing facility as well
as a storage garage.
Please advise if there is anything else you would need to deem this application complete.
