This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and "pre-existing" land uses:
1. Accurate survey of property showing all property lines, streets, building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is
denied, the Building Inspector shall state the reasons therefor in writing to the applicant.

Mr. Kassner:
Enclosed are two sets of plans for a Transfer Station for the
above.
The Plot Plan shows the location of small and large dumpsters,
storage trailers, storage bins, parking of trucks and cars
ready for disposal together where small and large dumpsters
are stored, ready for delivery.
Three (3) drums for the storage of transmission fluid, anti-
freeze and waste oil will be in an enclosed steel storage
to contain the contents of product with a roof over the area
to protect the drums and the collection of rain water.
If there are any questions, please call as we are ready to
file with all authorities.

Dear Mr, Sambach:
The following resolution was adopted by the Southold Town Planning Board
at a meeting held on Monday, September 11, 1995:
WHEREAS, Roy A. Schelin and Joanne K. Schelin are the owners of the
property Known and designated as North Fork Sanitation, located at Cox
Lane, Cutchogue, SCTM# 1000-83-3-4.7; and
WHEREAS, a formal application for the approval of this site plan was
submitted on February 27, 1995; and
WHEREAS, the Southold Town Planning Board, pursuant to the State
Environmental Quality Review Act, (Article 8), Part 617, declared itself lead
agency and issued a Negative Declaration on April 10, 1995; and
WHEREAS, this site plan was certified by Thomas Fisher, Senior Building
Inspector, on August 7, 1995; and
WHEREAS, all the requirements of the Site Plan Regulations of the Town of
Southold have been met; be it therefore

RESOLVED that the Southold Town Planning Board grant conditional final
approval on the surveys dated April 13, 1995, and authorize the Chairman

Dear Ms. Cheshire:
The following resolution was adopted at a meeting of the Southold Town Planning
Board on January 9, 2012:
WHEREAS, this site plan is for a proposed transfer station for construction demolition,
scrap metal, damaged cars; dismantling with shipping off site, refrigerators, stoves,
wood, aluminum; parking of sanitation trucks, trucks, cars and dumpsters; and
WHEREAS, on February 27, 1995, the applicant submitted an application for site plan
review; and
WHEREAS, on April 10, 1995, the Southold Town Planning Board, pursuant to the
State Environmental Quality Review (SEQR), (Article 8), Part 617, declared itself lead
agency and issued a Negative Declaration; and
WHEREAS, on September 5, 1995, the Southold Town Planning Board, pursuant to
Southold Town Code §280-131 C., distributed the application to the required agencies
for their comments; and
WHEREAS, on September 12, 1995, the Southold Town Planning Board granted
Conditional Final Approval on the surveys dated April 13, 1995, conditions to be met: (1)

Applicant must comply with recommendations of the Town Engineering Inspector as to

installation; it was determined that additional information regarding drainage provisions
was necessary to satisfy the conditions of approval; and
WHEREAS, on December 6, 2011, the Southold Town Engineer reviewed the above-
referenced application and has determined the project to provide adequate drainage
and the proposed drainage meets the minimum requirements of Chapter 236 for Storm
Water Management; and
WHEREAS, at a Work Session held on December 12, 2011, the Planning Board found
that all applicable requirements of the Site Plan Regulations Article XXlV, §280 - Site
Plan Approval of the Town of Southold have been met; be it therefore
RESOLVED, that the Southold Town Planning Board grants Approval to the site plan
entitled "North Fork Recycling", prepared by Sherman Engineering & Consulting, P.C.,
on September 27, 2011, last revised October 18, 2011, and authorizes the Chairman to
endorse the site plan.
Please note the following requirements in the Southold Town Code relating to site
plans:
1. Any outdoor lighting shall be shielded so the light source is not visible from
adjacent properties and roadways. Lighting fixtures shall focus and direct the
light in such a manner as to contain the light and glare within property
boundaries.
2. All storm water run-off from grading, driveways and gravel areas must be
contained on site.
3. Approved Site Plans are valid for three years from the date of approval, within
which time all proposed work must be completed, unless the Planning Board
grants an extension.
4. Any changes from the Approved Site Plan shall require Planning Board
approval.
5. Prior to the issuance of a Certificate of Occupancy, the Planning Board must
inspect the site to ensure it is in conformity with the Approved Site Plan, and
issue a final site inspection approval letter. Should the site be found not in
conformance with the Approved Site Plan, no Certificate of Occupancy may
be issued unless the Planning Board approves the changes to the plan.

Regarding the above referenced property, and our phone conversation this morning,
please consider this request to renew Building Permit//23012 issued September 13,
1995 to North Fork Sanitation in order for us to obtain a final inspection and obtain a
Certificate of Occupancy from the Planning Department.
In August, 2010 we began this process and at that time an Electrical Inspection was
conducted by Roger Richert which determined that no electrical permit was needed
for the scale on the premises. Robert Fisher conducted a Fire Inspection and advised
the owner to install three or four 20 lb. ABC fire extinguishers on the premises which
are now in place.
At that time, it was determined that the original 1995 Site Plan should be updated and
approved by the Town of Southold Planning Department. By Resolution dated
January 9, 2012, Site Plan Approval was granted and we have been informed by the
Planning Department that a copy of the Resolution and Site Plan Approval has been
forwarded to your office.
Additionally, on November 22, 2010, after an inspection at the site, the Suffolk
County Department of Health Services updated and approved their former Board of
Review Determination dated March 31, 1998. A copy of which is enclosed.
We have provided you with a check from North Fork Sanitation in the sum of $92.50
the requested fee for the Building Permit renewal. Please advise when a Final
Inspection can be completed.
