This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use: 1. Final survey of propertY with accurate location of all buildings, preperty lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of I% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Cmnpliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Mr. Vedty:
A visit was made to the above referenced property to evaluate the work performed at the site.
To the best of my knowledge, the fire walls are installed in accordance with UL Design No. U435
and to the best of my knowledge in conformance with the NYS Building Code. At the ceilings, a 1
hour ceiling extending 48" wide on each side of the wall or an 18~ wide 3 hour ceiling was
installed at each fire wall as required by the NYS Building Code.
In addition, the foyer and staircase to the second floor of Unit 6 is constructed with two layers of
sheetrock on the walls and ceilings with a minimum 1 hour rating with 1%" hour fire rated doors.
The walls are constructed equivalent or better to the UL design no. U301. The ceiling is
constructed equivalent or better to UL design no. P522. The minimum exit distance to the foyer
and staircase is less than 75' from any point on the second floor and the occupancy of the 2n~
floor is not more than 30; therefore, a second staircase is not required.
The minimum exit distance from the mezzanine to the exit door in unit 1 is less than 75' from any
location and the occupancy is not more than 30; therefore, a second staircase is not required.
To the best of my knowledge, the insulation installed in the ceilings and walls is in conformance
with the Energy Code of NYS.
To the best of my knowledge, the framing installed for the mezzanines and second floor are in
conformance with the code and capable of supporting the current uses of the building.
To the best of my knowledge, the plumbing appears to be installed in conformance with the
plumbing code based on the plumbing visible in the building.
To the best of my knowledge, the handicap accessible baths appear to bo installed in
conformance with the NYS Building Code.
The existing Quonset hut anchoring was installed in conformance with the plans and appears to
be structurally sound.

sets of plans, accurate plot plan to scale. Fee according to schedule.

addition six months. Thereafter, a new permit shall be required.
