YOU ARE HEREBY GIVEN NOTICE:
1. That it is the intention of the undersigned to request a Permit from the Board of
Trustees to:
Demolish and remove existing 2-story dwelling and appurtenances and construct-partially within Chapter 275
jurisdiction-a 2-story, one-family dwelling with attached garage, porches, decks, and patios, a new pervious gravel
driveway, and a drainage system of leaders, gutters, and drywells; remove existing sanitary system located less than
100 feet from wetlands and install new, upgraded sanitary system at least 135 feet from wetlands and thus beyond
Chapter 275 jurisdiction; and establish a variable width (minimum 50 feet), approximately 31,000 square-foot
nondisturbance buffer, all as depicted on the site plan prepared by Mark Schwartz and Associates, last dated March 2,
2010.
2. That the property which is the subject of Environmental Review is located
adjacent to your property and is described as follows:
9205 SKUNK LANE, CUTCHOGUE
3. That the project which is subject to Environmental Review under Chapters
96,111 and/or 276 of the Town Code is open to public comment on:

The above-referenced proposal is under review of the Board of Trustees of the Town of
