INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plaas, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. Tile work covered by this application may aot be commenced before issuaoce of Building Permit.
d. Upon approval of this application, tile Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on tile premises available fbr inspection throughout tile work.
e. No building shall be occopied or used in whole or in part for any purpose w'hat so ever until tile Building Inspector
issues a Certificate of Occupancy.
E Every building permit shall expire if the work authorized has not commenced within 12 mouths after the date of
issuance or has not been completed within 18 mouths from such date. If no zoning amendments or other regulations affecting tile
property have been enacted iu tile iaterim, the Building Inspector may authorize, in writing, the extension oftbe permit for an
addition six months. Thereafter, a new pennil shall be required.

PROPERTY FACT~/DESCRIPTION: Subiect parcel is improved with a one and two story dwelling with garage

square footage of 750 sq. ft. on lots 20,000-60,000 sq. ft.

AMENDED APPLICATION: During the hearing, the applicant was asked to bring the plan into more conformity 7,,
