A. For new building or new use: 1. Final survey of property with accurate location of all buildings, p.rope~ty lines, streets, and unusual natural or

2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate
of Code Compliance from amhitect or engineer responsible for the building.
6. Submit PI.arming Board Approval of completed site plan requirements.

a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Building Zone Ordinance of the Town of Southold, Suffolk County, New York, and other applicable Laws, Ordinances or

requirements listed in the letter, dated July 12, 2011, specifically: the removal of
portable signs, re-painting of traffic flow arrows and striping, and appropriate drainage
installation according to the as-built site plan have been completed based on the site
inspection made December 22,2011 and the site is now in conformance with the
approved site plan.
This letter does not condone any changes from the approved site plan and approvals

Based on our conversation, the Southold Town Building Department will lift the stop
work order issued against the Valero property in Cutchogue, NY (SCTM# 1000-102-5-
26), effective inunediately.
However, only work permitted under building permit #33893Z is permitted to continue.
Upon completion of construction, allowed under building permit #33893Z, your client
will be permitted to operate a gasoline station and automotive uses only. Any additional
proposed uses must first be approved by this office.

work order on the above referenced property as it relates to the rephcem~nt of gasoline
ta~ks and what/s otherwise allowable in the pcamt.

Section 220 Penalties
The heed of a state or municipal department, board, commission or office authorized or required by law to
issue any permit for or in connection with any work involving the employment of employees in employment
as defined in this article, and notwithstanding any general or special statute requiring or authorizing the issue
of such permits, shall not issue such permit unless proof duly subscribed by an insurance carrier is produced
in a form satisfactory to the chairman, that the payment of disability benefits for all employees has been
secured as provided by this article. Nothing herein, however, shall be construed as creating any liability on
the part of such state or municipal department, board, commission or office to pay any disability benefits to
any such employee if so employed.
