a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of Clans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e. No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim, the Building Inspector may authorize, in writing, the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Wood frame dwellings to be designed th accordance with American Society of Civil Engineers, Minimum
Design Loads for Buildings and Other Strueturas - ASCE 7-98. Wind speed to be used is 120 mph. Exposure
B category to be used for urban, suburban or wooded and Exposure C to be used for open and shoreline areas.
5) Concrete shall be air-entrained with an air content not less than 5% or more than 7%.
6) Concrete sltunp shall not be more than 5". Engineer has approval to reject concrete if slump is larger than
specified.
7) If concrete is delivered in an outdoor temperature lower than 40 degrees F it should be heated to insure that it
will arrive at the job site having a temperature of not less than 60 degrees F or greater than 85 degrees F.
8) If it looks as though the temperature of the ambient air will fall below freezing for 3 consecutive days, cold
weather concrete specifications will need to be followed. Contact Engineer if this is going to occur.

CONCRETE SPECIFICATIONS
1 ) Interior and exterior footings, slabs, and walls shall be 3000 PSI concrete.

FOUNDATION PILING NOTES
1 ) Pile notching not to exceed 50% of the pile cross sectional area.
2) Shimming of girders with shims larger than ¼" shall not be allowed.
3) Diagonal bracing shall have an end distance of 7 bolt diameters ~md an edge distance of 1.5 bolt diameters.
4) All bolts and washers for piles and bracing to be %" hot dipped g~alvanized or stainless steel washers to be
installed on both head and thread ends.
5) Bolt locations for all girders shall have an end distance of 4 bolt minimum diameters Edge distance shall not
exceed 4 bolt diameters at the top nftim girder and 1.5 bolt diameters at the bottom of the girder.
6) All work to conform to Coastal Construction Manual FEMA 55. Third Edition.
7) Ail girders are to be 3"x12" TREATED Southern Pine double notched into plies with aA" galvanized bolts
thru pilings. Bottom of girders to be at or above base flood elevation.
8) 3"x6' TREATED Southern Pine diagonal bracing is to be provided at a ration of one brace per piling - to be

9) All pilings are to be 1.0 PCF TREATED Southern Pine with minimum 8" diameter tip and 12" diameter butt.
